10101-Manually operated pilot ladder winch reels should be provided with a brake or other suitable arrangements to control the lowering of the pilot ladder and to lock the winch reel in position once the pilot ladder is lowered into position. PSCO observed the starboard side pilot ladder winch reel missing a brake and lock. Additionally, port side pilot ladder winch reel was missing mechanism to raise and lower pilot ladder, as well as missing brake and lock. 74 SOLAS(20) V/23.1.3 A.104S(27)7.5.3

09210-After any survey of the ship under regulations 7,8,9 or 10 has been completed, no change shall be made in the structural arrangements, machinery equipment, and other items covered by the survey without the sanction of the administration. PSCO observed heat lamp installed by the crew under the L/O purifier control panel. Chief engineer stated that the panel needs to be heated in order to start. Chief engineer also provided documentation from Alfal Laval technician stating that control panel needed to be replaced and recommended to be upgraded. However there was no documentation from the administration approving alternate arrangements. 74 SOLAS(20) I/11(b)

Resolution: USCG Sector Maryland-NCR received and reviewed the attached Class Report clearing the Vessel of the above deficiency and authorizing the vessel to utilize this alternative arrangement with a due date of 06FEB2023.
15 - Safety Management Systems (ISM) › N/A - No Subsystem › Maintenance of the ship and equipment

15109- The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. As stated in deficiency #1 PSCO Observed both pilot ladders not rigged in accordance with guidelines in resolution A.1045(27. Additionally, PSCO identified that the crew was not conducting the Pre-use inspections or monthly inspections as required by the SMS. Furthermore, the crew was not able to produce any evidence that the results of the inspections were recorded in the deck logbook or reported to the Master as required by the SMS. Due to the objective evidence in the above deficiencies, the vessel is not in substantial compliance with the relevent conventions. The Captain of the Port questions the adequacy and overall implementation of the vessel's SMS under ISM code. An internal audit is required within 30 days to determine whether the ship is operating in accordance with ISM code. Provide a copy of audit report to USCG when complete. 74 SOLAS(20) IX/3.1 ISM code 10.1
