THE THIRD VALVE SHOULD BE IN A PIPE THAT VENTS TO A SAFE LOCATION IN OPEN AIR, THAT PORTION OF THE GAS FUEL PIPING THAT IS BETWEEN THE TWO VALVES IN SERIES. THESE VALVES SHOULD BE ARRANGED SO THAT A FAILURE OF THE NESSECARY FORCED DRAUGHT, LOSS OF FLAME ON BOILER BURNERS, ABNORMAL PRESSURE IN THE GAS FUEL LINE, OR FAILURE OF THE VALVE CONTROL ACTUATING MEDIUM WILL CAUSE THE TWO GAS FUEL VALVES WHICH ARE IN SERIES TO CLOSE AUTOMATICALLY AND THE VENT VALVE TO OPEN AUTOMATICALLY. WHEN TESTING THE #1 BOILER DOUBLE BLOCK AND BLEED SYSTEM, THE VENT VALVE DID NOT AUTOMATICALLY OPEN WHEN THE TWO VALVES IN SERIES CLOSED WHEN EXPOSED TO 60% LEL SPAN GAS.

Each gas utilization unit should be provided with a set of 3 automatic valves. These valves should be arranged so that the failure of the nessicairy forced draught, loss of flame on boiler burners, abnormal pressure in the gas fuel supply line, or failure of the valve control acctuating medium will cause the two gas fuel valves which are in series to close automatically and the vent valve to open automatically. When testing a gas detector in the hood room to 60% LEL, the vent valve did not automatically open.

Gas protection equipment should be so designed that may readily be tested. Testing and calibration should be carried out at regular intervals. Suitable equipment and gas for purpose should be carried on board. When PSCO requested to test gas system in the SW2020 panel, crew applied 60% LEL methane span gas. System read 89% LEL. PSCO requested calibration, calibration failed on #1 sensor. System manual states that sensor must be replaced if it fail calibration. PSCO requested calibration of all 4 sensors. Sensor 3 failed calibration. Sensors 2 and 4 passed.

The ISM should ensure compliance with rules & regulations. The inspections mentioned in 10.2 and 10.3 should be incorporated into the ship's maintenance routine. Ship's ISM procedure for gas detection calibration states that gas detection calibration is to be checked in accordance with maker's instruction, on a monthly basis. Vessel crew have not been using maker's instructions to conduct calibration. Recommend an internal audit of the safety management system.

Fire fighting systems and appliances shall be kept in good working order and readily available for immediate use. During the examination of the compressor room on deck, the PSCO noted that the key to open the CO2 cabinet that protects the comporessor room was rusted out. The PSCO tried to open the cabinet with the rusted key and it broke without openign the cabinet.
