A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry—(1) Six hand red flare distress signals and (2) Six hand orange smoke distress signals. MI observed only 3 red flares on board vessel. Replace to the satisfaction of the attending MI.

A vessel not required to have a power driven fire pump must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. MI observed 02 missing fire buckets. Replace to the satisfaction of the attending MI.

Certification or licensing by a state or local jurisdiction as a fire extinguisher servicing agency will be accepted by the Coast Guard as meeting the personnel certification requirements of NFPA 10 for annual maintenance and recharging of extinguishers. MI observed 02 fire extinguishers with expired service tags/records. Replace/Repair to the satisfaction of the attending MI.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. MI observed 02 missing portable light. Replace to the satisfaction of the MI.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. MI observed emergency instructions missing from operating station. Replace to the satisfaction of the attending MI.

By March 15 of the year following the collection of the data in paragraph (a) of this section, marine employers must submit the data on the form titled U.S. Department of Transportation Drug and Alcohol Testing MIS Data Collection Form. MI observed missing MIS report. Replace to the satisfaction of the attending MI. 46 CFR 16.500 (b) (1)

The EPIRB test shall be logged or otherwise documented, as applicable. MI observed missing EPRIB entry logs. Provide evidence on documentation to satisfaction of MI.

Limited Coastwise (Not more than 20 miles from a harbor of safe refuge). Within three miles of shore. W/float free 406 MHz EPIRB. MI observed fault during EPIRB test. Vessel is restricted to Lakes, Bays, and Sound route only until functional EPIRB is installed.

46CFR176.500(b)(1) 701 c A vessel must undergo an inspection for certification as specified in 176.404 and annual inspection as specified in paragraph (b)(1) of this section. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel was due for an annual COI inspection on 15 March 2024. Vessel shall not operate with passengers until annual COI inspection has been completed.

46CFR176.600(c)(1) 701 c The owner or managing operator shall make a vessel available for drydock examinations and internal structural examinations. A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel was due for its hull exam on 31 March 2024. Vessel shall not operate with passengers until annual COI inspection has been completed.

Vessel was due for drydock examination on 31MAR24 and has yet to receive a drydock exam. 46CFR176.600(c)(1)

A vessel of at least 7.9 meters (26 feet) in length must be provided with individual bilge lines and bilge suctions for each watertight compartment, except that space forward of the collision bulkhead. MI was unable to witness operation of bilge pumps. 46 CFR 182.510(a)

A vessel not required to have a power driven fire pump must have at least three 9.5 liter (2.5 gallon) buckets with attached lanyard. The words "FIRE BUCKET" must be stenciled in contrasting color on each bucket. Vessel did not have any fire buckets on board. 46 CFR 181.610

To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. EPIRB registration was not up-to-date to reflect the current owner. 47 CFR 80.1061(f)

Each life jacket must be marked in clearly legible block capital letters with the vessel's name. All life jackets on-board had the name of the previous vessel. 46 CFR 185.604(b)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel did not have a log where man overboard, abandon ship, fire drills, and EPIRB tests were logged. 46 CFR 185.520(f)

Vessel had emergency instructions for the previous vessel. 46 CFR 185.510(a)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel had an FCC Station License, but it was issued to previous vessel. 46 CFR 184.502

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. The owner repowered the vessel from a 200 HP Evinrude to a 250 HP Yamaha in 2018 without notifying the CG. The vessel's stability letter is based on a deadweight survey. A change in the lightship as per the stability letter requires a new stability test. The owner shall submit lightship characteristics to MSC for the repower. 46 CFR 185.100

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. The port hull was found with an unapproved repair located on the hull next to the port aft flush deck screw down. The repair was made with epoxy putty and does not match the approved FRP layup. The repair appears watertight and is located approximately 2" below the main deck. The repair measures approximately 4" in length and 3" in height. Permanent repairs shall be made prior to the next credit dry dock. 46 CFR 176.700

A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessel was due for COI inspection on 23FEB23 and has yet to receive a COI inspection. Vessel shall not operate with passengers until new COI has been issued. 46 CFR 176.100

A vessel must be provided with bilge pumps in accordance with Table 182.520(a). The vessel is arranged with a submersible electric bilge pump and a hand portable pump. The hand portable pump was not operational when tested by the owner. 46 CFR 182.520(a)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The owner purchased a new VHF radio and it was found installed at the operating console. The radio electrical conductors were missing enclosures or incorrectly installed enclosures. The VHF radio was not inspected by a FCC technician and a valid FCC safety radio certificate was not onboard. 46 CFR 184.502

Except as provided in paragraph (b) of this section, the weather deck on a flush deck vessel must be watertight and have no obstruction to overboard drainage. The aft freeing ports used for draining the main deck were found blocked by bundled electrical conductors on the starboard side and an anchor on the port side. Clear the freeing ports to permit free drainage aft. Remove the water found in the port forward hull compartment. Rectify prior to issuance of the COI. 46 CFR 178.410

Electrical equipment on a vessel must be installed and maintained to: Provide services necessary for safety under normal and emergency conditions; Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; Minimize accidental personnel contact with energized parts; and Prevent electrical ignition of flammable vapors. The DC electrical system was found with multiple dead end live positive conductors connected to the power source which presents a fire hazard and shock hazard to personnel. The positive conductors for the branch electrical components were all found attached together using multiple ring terminals connected by a rivet which presents a resistance and heat hazard. The negative conductors for the entire DC system were found connected to a screw and free floating behind the bulkhead. The positive and negative conductors for the DC system were not found connected to the terminal strips attached at the bulkhead behind the operating station. Install the DC electrical system as per original approved electrical drawings. Rectify prior to issuance of the COI. 46 CFR 183.200

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. A 25 amp blade fuse was found installed as the primary branch overcurrent protection next to the batteries which is not in accordance with the original approved electrical plans. The batteries were found wired in parallel and the CCA delivery is 1800 amps. The AIC for the 25 amp blade fuse is less than the CCA output of the batteries. Blade fuses are not authorized on commercial passenger vessels. Authorized UL listed cartridge fuses are RK-5, K-5, CC, or T. Provide a UL listed cartridge fuse to protect the 12 AWG conductor used to provide power to the DC electrical circuit. Rectify prior to issuance of the COI. 46 CFR 183.380(l)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. The master did not have a deckhand in attendance for the COI in order to complete a man overboard recovery drill. Rectify prior to issuance of the COI. 46 CFR 176.808(g)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A space with a through-hull fitting below the deepest load waterline. Prove operation of the high water alarm during the next MI attendance after the DC electrical system is repaired. 46 CFR 182.530

Port catamaran portable bilge pump was inoperable. Each catamaran on a vessel must have an operable bilge pump. 46CFR182.520(j)
