An offshore tank barge with a cargo capacity of 250 or more barrels that is carrying hazardous material as cargo must meet paragraph (a) of this section or be equipped per paragraph (b). Barge is not equipped with a primary containment at each cargo connection. Secondary containment was missing mechanical plugs according to 33 CFR 155.310(a)(3).

All cargo tank hatches must be clearly marked in not less than three inch lettering "Danger - Keep Out"... Cargo tank hatch markings were no longer visible on all 6 tank hatches. Cite: 46 CFR 35.05-15(b)(3)(ii)

An oil tanker of 150 gross tons shall also maintain an oil record book part 2 (cargo/ballast operations). Entries shall be made in the Oil Record Book on each occasion, on a tank to tank basis if appropriate, whenever any of the following cargo/ballast operations take place on any oil tanker to which this section applies: see (e)(1)-(11). Each completed operation shall be signed by the person in charge of the operations concerned and each completed page shall be signed by the master or other person having charge of the ship. Vessel's Oil Record Book Part II entries were erroneously recorded in ORB Part I. ORB Part I was also missing required entries.

Barge is not equipped with a container at either discharge manifold that meets 33 CFR 155.310(a) or 33 CFR 155.310(b).

The vessel operator of each vessel required by § 155.720 to have transfer procedures shall maintain them current. Vessel’s transfer procedures are not updated with all information required by 33 CFR 155.750 to reflect recent installation of cargo pump and associated piping.

A vessel owner or operator required to have a response plan shall conduct exercise as necessary to ensure that the plan will function in an emergency. The following are the minimum exercise requirements: Qualified individual notification exercises, which must be conducted quarterly; emergency procedure exercises, which must be conducted quarterly. Vessel could not provide records of quarterly QI notifications or emergency procedure exercises.

Each oil tanker of 150 gross tons and above shall maintain an oil record book part I (Machinery Space Operations). Vessel does not maintain an ORB Part I. Rectify prior to conducting operations required to be recorded in ORB Part I.

An oil tanker of 150 gross tons shall also maintain an oil record book part 2 (cargo/ballast operations). Vessel's Oil Record Book lacked accurate and valid entries as well as required signatures. Rectify prior to conducting operations required to be recorded in ORB Part II.

The VSO must ensure that at least one security drill is conducted at least every 3 months Vessel could not provide documentation of security drills.

Exercises must be conducted at least once each calendar year, with no more than 18 months between exercises. Vessel could not provide documentation of most recent company security exercise.

The CSO or VSO must ensure an audit of the VSP is performed annually, beginning no later than one year from the initial date of approval. For each annual audit, a letter certified by the CSO or the VSO stating the date the audit was completed. Vessel could not provide documentation of recent audit of security plan.

Each vessel owner or operator subject to this part must ensure that copies of the following documents are carried on board the vessel and are made available to the Coast Guard upon request: the approved Vessel Security Plan (VSP) and a letter of approval from the Commanding Officer, Marine Safety Center (MSC). Vessel could not provide signed letter of approval from MSC for VSP. VSP cover page indicated a previous approval.

The starboard hull deck line was not visible externally. The deck line must be painted prior to service and permanently marked prior to completion of next drydock in accordance with Load Line Regulations.

The forward port navigation light stanchion was considerably bent. Make repairs to ensure navigation lights can be properly mounted prior to operating in restricted visibility.

Barge was missing Oil Pollution Prevention Placard.

Vessel transfer procedures were missing required information.
