IAPP CERT shall be issued after a renewal survey in accordance with the provision of regulation 5 of this annex. Vessel did not have access to their tech file and EIAPP for all engines listed on current IAPP and missing record book. Vessel shall provide copies and/or proof prior to getting IAPP endorsed. CITE: MARPOL ED. 2022, ANNEX IV, REG 6.1.1

Oil Record Book Part 1 shall be completed on each occasion, on a tank-to-tank basis, whenever any of the machinery space operations takes place. Crew had improper record keeping of waste oil and ballast water to reception facilities. Also not logging collection of oily water and ballast water in tanks. Company shall provide proper training and provide proof of completed training. CITE: MARPOL ED. 2022, ANNEX I, REG 17.2

Maintain ballast water management plan that has been developed specifically for the vessel. Vessel shall retain a vessel specific ballast water management system plan and implement it, to include record keeping. CITE: 33 CFR 151.2050(g) 33 CFR 151.2070(a)

Each self-contained breathing apparatus shall have a minimum 30 minute air supply. Vessel shall maintain air bottles to be full with fire suits. CITE: 46 CFR 96.35-5(b)

If a lifebuoy is intended to operate the quick release arrangement provided for the self-activated smoke signals/ lights, it shall have a mass sufficient to operate the quick release arrangement, but not less than 4kg (8 pounds 13 ounces). The lifebuoys in this arrangement were regular deck buoys and not of adequate weight. Vessel shall install proper bridge buoys. CITE: 46 CFR 199.150-4(e)

The Oil record book part 1, shall be completed on each occasion if appropriate whenever collecting and disposal of oilresidues. Crew was not recording weekly soundings

Each discharge operation shall be recorded in the garbage record book. Crew was not logging plastic discharge.

Each vessel must minimize hazards (fire). Broken bracket on STBD MDE high pressure fuel line.

Each Vessel must comply with 47CFR part 80. Radiotelophone log was not being checked and signed by master.

Each liferaft must be arranged for float free launching. Vessel float free arrangement was unsatisfactory.

Each liferaft must be arranged for float-free launching; liferafts were not properly attached to weak link to provide float free arrangement.

Each class 1 door on a vessel in ocean service must be cabals of holding head of water. Door to emergency escape hatch on starboard side needs gasket replaced. Starboard emergency escape hatch needs gasket replaced and readjustment. Battery stuffing tube on port side to be replaced to ensure watertight integrity.

Vessel must have line-throwing appliance approved under 160.040. Incorrect line-throwing appliance on board and associated equipment. Must have impulse style.

Every ship of 400 GT and above shall be provided with an Oil Record Book Part 1 and shall be completed on each occasion when machinery space operations occurs on vessel. Vessel did not have Oil Record book.

Vessels 400 GT or greater should maintain Garbage Record Book. The Garbage Record Book had incorrect entries, corrections with whiteout, and lack of signature on each page.

Detectors must be responsive to heat, smoke, or products of combustion. Smoke detector in #4 crew room inoperable, to be repaired/ replaced.

Connection on all firefighting extinguishing equipment to be checked to ensure it’s in operating condition. Canister in CO2 room needs but tightened to hose.

46 CFR 160.156-7: Exterior of each rescue boat must be marked with Type II retro reflective material. Type II retro on rescue boat excessively worn. Starboard aft ring buoy has lettering over Type II retro, to be repaired.

Powerdriven vessels underway should have sidelights. 01 starboard nav. light inoperable; replaced on spot

Lighting for survival craft launching stations required. Starboard life raft emergency light inoperable.

Every vessel over 400 GT shall have the bunker delivery note kept onboard the vessel NLT three years and the bunker sample shall be kept onboard until the fuel oil is substantially consumed, but NLT 12 months after delivery. Vessel did not have bunker notes for the previous 3 years nor fuel samples for the past 12 months. MARPOL VI/18.5 & MARPOL VI/18.8.1

Vessel shall carry the personnel requried by the COI. The vessel did not ahve the required lifeboatman (2) and deckhands (2) as the Captain did not the the lifeboatman and one deckhand had an expired TWIC w/o credentials.

Rescue boat shall be in good working order and ready for immediate use. Davit system was not able to recover rescue boat.

Vessel shall preforme quartly inspectrion and maintaince of rescue boat with MOB. Vessel shall preforme MOB with Rescue boat or rescue platform. Vessel did not preform MOB with rescue boat.

Lifefloats shall be outfitted IAW 46CFR106.073. Lifefloats did not have the required reflective tape.

Vessel shall maintain up to date charts. Vessel had not updated charts since printing on May 10, 2021.

Vessel shall have an opperatonal rudder angle indicator. Vessel's angle indicator was not operational during non-followup for Stbd rudder.

A rudder angle indicator independent of the control of the main steering gear must be installed at the steering control station in the pilot house. Rudder angle indicators at forward steering station in pilot house indicate a difference of 5 degrees from each other when operationally tested. Rudder angle indicators at aft steering station in pilot house indicate a difference of 10 degrees from each other when operationally tested.

All ships of 400 gross tonnage and above and every ship which is certified to carry 15 persons or more engaged in voyages to ports and offshore terminals under the jurisdiction of another Party to the Convention shall provide a Garbage Record Book to record all disposal and incineration operations. A new form of the Garbage Record Book (GRB) has been included in appendix II to MARPOL Annex V. Each ship must comply with Annex I, II or V of MARPOL 73/78 unless otherwise indicated. The vessel does not have onboard the new form of Garbage Record Book to engage in voyages to ports and offshore terminals under the jurisdiction of another Party to the Convention.

Each ship that is operated under the authority of the United States and is certificated for ocean service or coastwise service beyond 3 nautical miles from land shall maintain an oil record book in accordance with 33 CFR 151.09(a). Entries shall be made in the oil record book on each occasion when disposal of bilge water or oil residue that has accumulated in the machinery spaces takes place. The oil record book is missing all entries for removing bilge water or oil residue from machinery spaces since the vessel began operating under its certificate of inspection. The operator admitted to offloading bilge water and oil residue from the machinery space to their own facility which is not an approved reception facility to dispose oily waste or contaminated bilge water from vessel machinery spaces. One receipt was available for offloading oil, oily waste, or contaminated bilge water from vessel machinery spaces. The receipt was issued by PENCO in May 2017.

No person may operate an oceangoing ship of less than 400 gross tons on an international voyage unless it is fitted with an approved 15 parts per million oily water separating equipment for processing oily mixtures from bilges. Equipment installed after 2004 must meet current standards in 46 CFR 162.050. Oil content meter found installed onboard without a calibration certificate.

Annual IOPP surveys for inspected ships are to determine that the oily water separating equipment and associated pumps and piping systems remain satisfactory for the service intended, and that no unauthorized alterations have been made. A licensed chief engineer was not present for the annual inspection. The crew onboard was not familiar with the OWS operating procedures. A piping diagram with the original approved oily water separating equipment and associated pumps and piping systems was not onboard. The marine inspector is unable to determine if unauthorized modifications have been made without the original approved diagram to review. Make oily water separating equipment available for inspection by a competent licensed engineer officer per the COI.

Provide an approved D/C electrical diagram within 30 days as per 46 CFR 90.25-1.

Prove the proper installation of the A/C electrical system within 30 days as per 46 CFR 90.25-1.

Provide updated navigational charts for all areas of operations within 7 day as per 46 CFR 97.05-5.

Prior to departure and at all times necessary the master shall ensure that the vessel complies with all applicable stability requirements and log the vessel’s draft, trim, and stability in the unofficial logbook as per 46 CFR 97.15-7.

No means shall be provided to lock doors for the required means of escape. Prove no means of escape can be locked to prevent escape within 30 days as per 46 CFR 92.10-20.

Prove the proper operation of the lube oil, over speed, and high cooling water temp alarm and shutdowns within 30 days as per 46 CFR 61.20-3.

Provide embarkation ladder for both embarkation stations on 2nd deck as required by 74 SOLAS 2014, Chapter III, Regulation 11.7

Replace all valves and check valves constructed of bronze or other non-ferrous materials which are installed in fuel oil, lube oil, dirty oil, and oily water systems at next dry docking.

provide proper over-current protection for the emergency power feeder to the navigation light panel (conductor provided AC power from the inverter to the nav light panel). Over-current protection must be designed to protect the conductors (UL Listed 489) & meet the requirements of 46CFR 111.50-2, 111.50-3, 111.50-5, & 111.75-17(a).

Provide low-voltage release for fire/bilge/ballast pump #2 motor contactor.

Label all DC power panels and circuits IAW the USCG approved one-line diagrams.

Ensure over-current protection of steering motor is in accordance with 46CFR 58.25-55

Provide steering alarms which meet all the requirements for operation, alarm type, & the location of audible and visual indicators required by 46CFR 58.25-25(d) 1 thru (d)3 and (e)1 thru (e)3

Install Bridge, Navigation, Watch, Alarm System (BNWAS) as required by 74 SOLAS 2014, Chapter V, Regulation 19,2.2.3.3.5

Provide spare magnetic compass that is interchangable with the ships primary magnetic compass as required by 74 SOLAS 2014, Chap V, Regulation 19.2.2.1

Repair leak on OWS water pump

Provide current copies of the Internation Code of Signals and IAMSAR Manual as required by 74 SOLAS 2014, Chapter V, Regulation 21

Provide updated compass deviation table. New equipment was installed on the vessel after date listed on deviation table.

Provide approved first aid kit. Items in current kit were expired.

Provide SOLAS approved lights for all lifejackets.

Demonstrate operation of rescue boat.

Provide paper copies of all CG approved plans and comment letters.

Make engine technical files & applicable international air pollution prevention documents available for inspection.

Complete annual inspection no later than 11 August.
