Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Portable fire extinguisher in starboard machinery space not mounted. Electrical equipment on a vessel must be installed and maintained to: minimize accidental personnel contact with energized parts. Multiple xposed busbars underneath operating station. Provide suitable protection to the satisfaction of the attending marine inspector. 46 CFR 183.200(c)

The master of the vessel shall ensure that: Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; and The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Vessel unable to produce evidence of EPIRB testing in logbook. Provide evidence of monthly EPIRB testing to attending marine inspector. 46 CFR 185.728(a) & (c)

Two areas of damage were discovered in the vessel's port hull. Once the size of the damaged area is determined, submit a repair proposal for approval to the attending Marine Inspector. Once the repair proposal is approved by the attending Marine Inspector, complete the repairs to the satisfaction of the attending Marine Inspector. 46 CFR 176.700(a)

All machinery must be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Cooling water hose on port engine found resting on engine exhaust pipe. Reposition hose or provide heat resistant shielding protection to the satisfaction of the attending Marine Inspector. 46 CFR 182.01-5(a) "Old T"

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel did not have radio station authorization. Provide to the satisfaction of the attending Marine Inspector. 46 CFR 184.502

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The captain employed by company did not have a medical certificate. Provide valid medical certificate to the attending Marine Inspector (MI) before being employed in a position where a person employed in the service of a vessel is considered to be acting under the authority of a credential or endorsement when the holding of such credential or endorsement is: (1) Required by law or regulation; or (2) Required by an employer as a condition for employment. 46 CFR 10.301(a)

Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel did not complete annual inspection within 3 months of the anniversary date. Vessel prohibited from carrying passengers until a satisfactory COI inspection is completed. 46CFR 176.500(b)(1)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Engine room pvc supply duct piping is duct taped to an electric cable for support. Remove duct tape and properly support piping. 46 CFR 176.830(a)

No marine employer shall engage or employ an individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Vessel Captain has not completed a chemical test prior to employment with the company. Provide proof that the Captain has passed a recent chemical test for dangerous drugs or has passed a test within the previous six months. 46 CFR 210(a)

A vessel must have one or more ring life buoys that are approved IAW 46 CFR 160.050. Ring life buoy grab line is severely degraded. Replace grab line on ring life buoy and install to the satisfaction of the MI. 46 CFR 180.70(b)(1)

Each ring life buoy with a floating waterlight must have a lanyard, securing the waterlight around the body of the ring life buoy. Each floating waterlight must be attached to the lanyard with a corrosion resistant clip. Waterlight is not attached to the ring life buoy and the installation was missing a corrosion resistant clip. Install corrosion resistant clip on waterlight lanyard and properly secure to the body of the ring life buoy. Vessel restricted to daylight operations only. 46 CFR 180.70(d)(2)&(3)

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation. Vessel radio screen is inoperable as it does not display the radio channel. Replace radio installation and demonstrate proper operation to attending Marine Inspector. Vessel can not operate more than 1,000 feet from shore. 47 CFR 80.905(a)(1)

Vessel owners shall advise NOAA in writing upon change of vessel or EPIRB ownership, transfer of EPIRB to another vessel, or any other change in registration information. Vessel owner was unable to provide documentation or registration showing the EPIRP ownership and contact information has changed to the new ownership. Provide documentation showing EPIRP information has been updated to the new owner. 47 CFR 80.1061(e)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. The fire bucket on board were only 2 gallons and two were missing lanyards. Provide fire buckets with proper capacity and appropriate lanyards. 46 CFR 181.610

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position or perform the duties and functions of a position, required by the vessel's COI. Marine employers may form or otherwise use sponsoring organizations, or may use contractors to conduct the random chemical testing programs. Vessel's O/O is developing a chemical testing program to include random-testing by third party, but has not completed the program. Provide satisfactory evidence of a chemical testing program. 46 CFR 16.230(a)

Unless otherwise stated in this section, life jackets must be stored in convenient places distributed throughout accommodation spaces. Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Life jacket stowage not properly labeled. Label life jackets storage ensuring separation of adult and children life jackets. 46 CFR 180.78 (a)(5)

The owner shall be prepared to conduct tests and have the vessel ready for inspection of the following: On a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Vessel does not have in place a preventative maintenance plan to adequately assess the safe condition of their sail rigging system in accordance with USCG NVIC 02-16. The vessel does not have preventative maintenance plan. Provide preventative maintenance plan. 46 CFR 176.802(a)(3)

One ship station license will be granted for operation of all maritime services transmitting equipment on board a vessel. Vessel representative was unable to provide proof that Station License had been transferred to new owner. Provide proof Station License has been transferred to new owner. 47 CFR 80.13(b)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Provide a means of closure for the vent openings leading to the port and stbd engine room. 46 CFR 182.465(h)

Exhaust piping shall be led to the point of escape with a minimum number of bends or elbows. It shall be gastight throughout its length and shall be properly supported by non-combustible hangers or blocks. Exhaust leak detected in the port & stbd side engine exhaust. Ensure exhaust is gas tight throughout its length. Repair to the satisfaction of the attending Marine Inspector within 5 days. 46 CFR 182.20-20; 46 CFR 182.15-20 (a) Old T

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Port main engine accessory drive belt is incorrect belt size.Replace with proper sized belt. 46 CFR 176.800(a)

All equipment, including switches, fuses, lamp holders, etc., must be suitable for the voltage and current utilized. Fuses for bilge pumps are rated for 15A, but have 5A fuses. Replace with properly rated fuses. 46 CFR 183.220(b)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Marine Inspector noticed excessive vibrations in the starboard shaft. Realign starboard shaft and MI to witness operation. 46 CFR 182.05-1(a) Old T

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. The compartment where the life jackets are stored was labeled on the underside of the cover. Label the topside where the labeling can be read by passengers during an emergency. 46 CFR 185.604(f)

Placards containing instructions for the donning and use of life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel has two types of life jackets, but only one placard type. Post additional placard for the other type of life jacket. 46 CFR 185.516(a)

All portable fire extinguishers installed on vessels shall be of an approved type. The fire extinguisher at the operator's station approved mounting bracket is broken. Replace in-kind. 46 CFR 181.30-5(a) Old T

Flexible hose may be used provided it is fitted with suitable connection fittings and has high resistance to saltwater, petroleum oils and vibration In both engine compartments, the vent and fuel tank fill hoses are not labeled to determine if they meet the regulation. Provide evidence the meet the regulation or replace with flexible hoses that meet the J1527 standard. 46 CFR 182.20-30(d) Old T

The employer shall provide an Employee Assistance Program for all crew members that includes both education and training on the effects and consequences of drug and alcohol use. Supervisory personnel must receive at least 60 minutes of training. Vessel's owner/operator to establish an Employee Assistance Program (EAP) as described in the regulation. Provide EAP that meets the elements of the regulation for review and to the satisfaction of the attending MI. 46 CFR 16.401

Vessel needs to conduct a third party rigging survey by a recognized Rig Smith to include all sail rigging components on deck and aloft. All areas found to be out of service and compliance need to be renewed to serviceable conditions and verified by an attending USCG Marine Inspector.

Vessel needs to post lifejacket donning placard.

Vessel needs to have a complete first aid kit onboard.

Vessel needs to properly secure the portside engine room battery bank.
