Where the atmosphere in double hull spaces cannot be reliably measured using flexible gas sampling hoses, such spaces shall be fitted with permenant gas sampling lines. The configuration of gas sampling lines shall be adapted to the design of such spaces. PSCO discovered ballast tank gas sampling lines on deck were severed at cargo holds no.3 port and no.2 port, rendering the permenant gas sampling lines for ballast tanks no.1 through 3 port and starboard inoperable.

PSCO discovered exposed and loose wiring for pressure sensor for the tank cleaning heater and no.1 cargo pump bottom temp sensor in the cargo pump room.
