During the inspection noted multiple CO2 portable fire extinguishers missing pressure gauges. Reviewed NSE for the monthly job assignment for inspecting portable fire extinguishers but there were no procedures, instructions, or checklists on how to inspect this equipment. There is also no procedure within the SMS / JIBE.

• Only a copy of the ISSC was on board; the original certificate could not be located. The DNV Stability Declaration dated 2017-04-10 was not provided during inspection.
• In 2019, a crewmember changed the Lightship calculations for required DNV quarterly stability loading tests in 2019 with no reviewed or approved authorization(s).
• Multiple obsolete documents were found in the certificates binder (Certificate of Documentation, Non-tank Vessel Response Plan approval letters, and SOPEP approval letters). The Garbage Record Book (Garbage) had over three years of log entries and the provided Garbage Record Book dates did not correspond with SMS/JIBE.
