At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil that might create a fire hazard or clog bilge pumping systems. Leaking fuel oil was found in the bilges on during a Living Marine Resources boarding on 10APR24. After actions were taken to attempt to rectify that but failed, leaking fuel oil, both atomized and liquid, was again identified during an inspection on 19APR24, creating both pollution and fire hazards.

The vessel BARRACUDA is a SPV-T, therefore not required a safety management system (SMS) per regulation. Sector Key West has found the below deficiencies, in addition to the recently issued code 17c deficiencies on 11APR24(see MISLE activity 7898338) and 14APR24 (the instigating action for this inspection), to be objective evidence that a serious substandard condition has not been pro-actively managed by the vessel's owner/ operator, leading to a lack of safety culture on board the vessel.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel suffered propulsion system failure while underway requiring towing back to mooring. Determine cause of failure and make repairs to the satisfaction of the attending marine inspector.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, or debris, or other matter that might create a fire hazard or clog bilge pumping systems. Accumulation of oil found in engine room bilge with absorbent cloths. Remove oil and cloths, determine sources of leak and make repairs to the satisfaction of attending marine inspector.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not coverd by standards in this subchpater may be inspected in accordance with sandards acceptable to the cognizant OCMI as good marine practice. Main propulsion engine's shaft penetration was found to be leaking sea water at a steady stream into the bilge. Make adjustment or repairs to the main propulsion engine's shaft seal to the satisfaction of the attedning marine inspector. Corrected on the spot.

Each small battery installation must be located in a well-ventilated space and protected from falling objects. Both battery boxes were found with no terminal protection from falling objects. proveide a means of protection from falling objects and remvoe stored compoennts and debris from the falling battery box to the satisfaction of the attending marine inspector.

Anchor light does not work and also is not mounted in a location that can be seen from 360 degrees. Must make repairs to anchor light and move to top of vessel where it can be seen from all around.

Fuel supply and return hoses do not meet the SAE J1942 standard. Must replace both hoses that meet the requirements for CG approved SAE J1942.

Two stanchions on the aft railing are broken. Replace both stanchions.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. New fuel supply piping was installed without notifying the cognizant OCMI.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard or clog bilge pumping systems. Approximately 03 gallons of hydraulic fluid were found in the forward part of the lazarette bilge. Remove hydraulic fluid accumulation from the lazarette bilge. Identify source and make repairs.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected, and all required guards and protective devices must be in satisfactory condition. The following fire hazards were identified in the vessel’s main machinery space and shall be resolved to the satisfaction of the of the attending marine inspector: 1. The bilge was found to have super saturated oil soaked absorbent pads located under the main propulsion engine. Remove oil-soaked absorbent pads and all oil residue from the bilge. 2. An open lid bucket with approximately 02 gallons of diesel fuel on the port side of the main engine was present. Remove hazard. 3. Numerous bottles of compressed flammable materials were stored throughout the engine space. Remove hazard. 4. The main propulsion engine’s crank case vent was found to be improperly installed and venting directly onto the engine’s main air intake filter resulting in filter oil saturation. Properly install the main propulsion engine’s crank case vent.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Main propulsion engine’s shaft penetration was found to be leaking sea water at a steady stream into the bilge. Make adjustment or repairs to the main propulsion engine’s shaft seal.

Electrical equipment on a vessel must be installed and maintained to prevent electrical ignition of flammable vapors. Battery system terminal board found mounted directly to the main engine’s metallic fuel supply line. Remove terminal board from its current location and install in a suitable location.

All cable and wire must be protected by metal coverings or other suitable means if in areas subject to mechanical abuse. The positive cable for the main engine’s starting system was found to be pinched between deck grating and the hull and installed without protection across a stepway near the front of the engine. Install protection on the main engine’s positive starting cable.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Multiple exposed electrical wire connections found in underneath the main engine, in the overhead in the port aft section of the engine room and in the starboard overhead of the lazarette. Place all connections within a suitable enclosure.

Nonmetallic piping must not be used in gasoline or diesel fuel systems unless they meet SAE J1942 and J1475 specifications. Flexible fuel hose installed for the main engine fuel supply line does not meet SAE J1942 and J1475 specifications standards. Replace fuel supply line with materials meeting required standards.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The vessel’s port fuel tank located in the lazarette was found to have an inoperable shutoff valve installed. Repair or replace the inoperable fuel shutoff valve.

A space containing propulsion machinery must be equipped with a fixed gas fire extinguishing system. The indicator light for the pre-engineered fixed gas fire extinguishing system shows the system has been discharged. Determine and address the cause of the illuminated discharge light. Demonstrate proper system operation to satisfaction of marine inspector.

Pre-engineered fixed gas fire extinguishing systems must be capable of manual actuation from outside the space. The disabling pin for manual activation cable found installed at the cylinder end within the space being protected, preventing manual activation of the system from outside of the space. Remove disabling pin. Demonstrate proper system operation to satisfaction of marine inspector.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have a means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. Unable to test the function of the installed reset device due to system condition. Demonstrate proper operation of the vessel’s fixed gas fire extinguishing system automatic shutdown and reset device in the attendance of a marine inspector.

Except for engine exhausts, each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 150 millimeters (6 inches) above the deepest load waterline must have means to prevent water from entering the vessel if the pipe fractures or otherwise fails. Hull penetration in the forward starboard bilge of the engine room has a valve installed but no actuation handle. Install means to secure valve.

Each small battery installation must be located in a well-ventilated space and protected from falling objects. Both battery boxes were found with no terminal protection from falling objects. Additionally, electrical components and debris was found stored on top of the aft battery near the battery posts. Provide a means of protection from falling objects and remove stored components and debris from the battery box.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Guards are not installed over the main engine’s rotating machinery and belts. Install suitable guards.

Each life float must be fitted with a light. The light on the vessel’s starboard life float was found to be non-operational. Provide an operational light for the starboard life float.

Missing Crew training records/logs. Must show proof of crew training for each Captain and Mate prior to carrying passengers.

Missing logs for fire, MOB and abandon ship drills. Must provide proof of crew completely all drills prior to carrying passengers.

Vessel had 4 valid red flares and 4 expired flares. Vessel is required to have 12 flares onboard. Must replace missing 8 flares prior to carrying passengers.

Anchor/all around light, is mounted at the operating station under the canopy and light was not working. Must move anchor light to the highest point of the vessel so it can be seen from every direction. Vessel restricted to daylight hours only.

Life ring floating light was not onboard the vessel and not working. Must make repairs and place onboard the vessel. Vessel restricted to daylight hours only.

Life ring had a few tears into the foam, grab line was dry rotted and ring was faded. Must make repairs or replace life ring.

Missing Coast Guard approved Nav Chart. Must have updated Nav chart onboard or have GPS updated with latest Nav Chart.

Both life floats are fadded and need repainting. Must repaint both life floats.

Missing logs for monthly EPRIB test Must provide proof for testing EPRIB monthly.

MDE was overhauled and need to conduct engine test. There was no sensor for oil psi. Make repairs and conduct test to the satisfaction of attending CG Marine Inspector prior to carrying passengers.

MDE engine was overhauled and must conduct successful sea trials prior to carrying passengers.

Both automatic bilge pumps indicators at helm station were not working. Must make repairs and test to the satisfaction of attending CG Marine Inspector

Engine room ventilation does not close all the way. Must make repairs and test to satisfaction of attending CG Marine Inspector.

MDE guard was missing. Must have guard around all rotating machinery to prevent injury. Replace missing guard.

Engineroom was found with a lot of loose dead end wires hanging. Either connect all wires or pull out all dead end wires.

Most of the wiring in the engineroom were not supported and hanging down. Wiring must be supported with metal brackets at least every 24 inches.

Life jacket stowage needs re labeling and with correct qty.

All 4 life float paddles needs vessel's name written on them.

2 adult life jackets needs repair. Passenger count reduced by 2 until repaired or replaced.

Life jacket doning placard was missing. Must replace.

Emergency fuel shut off valves needs labeling.

Un able to conduct fire and MOB drills due to condition of vessel. Must conduct drills prior to carrying passengers.

EPIRB registratation and battery expired Sept 2020. Must register EPIRB and install new battery. Vessel is restricted to 3 miles from shore.

Vessel was found operating illegally with passengers with out a valid COI. Must conduct a COI inspection and issue a COI prior to carrying passengers.

FCC documentation missing for vessel.

Lifefloats were severely faded and retroreflective tape was no longer servicable. Repair/replace lifefloats.

Lifefloats are not connected to vessel via the weak link. Install painter/weak link to vessel in accordance with regulations.

Alarms for both the high water and automatic bilge pump are not properly labeled and are not properly functioning. Repair and relabel all bilge pumping and alarm arrangements.

Vessel through hulls and starboard fuel tank are not properly grounded. Ensure all required components of vessel are properly grounded to prevent accelerated corrosion.

Vessel is in the process of replacing VHF radio. Ensure radio installation is completed in accordance with regulations.

Vessel has multiple dead end wires located in the engine space. Remove all wiring that is not currently being utilized by the vessel.

Lifering found to be unservicable. Operator replaced life ring prior to departure of MI.

07 lifejacket light batteries were dead. Batteries replaced on the spot.

Fireboy system was not serviced. Ensure system is serviced. Following servicing a test of the machinery shutdowns and override function must be successfully completed in the presence of a marine inspector.

Man Overboard Drill must be successfully completed in the presence of a marine inspector.

Replace and/or sister the damaged 4th and 5th stringers from aft on starboard side with materials to meet the structural strength requirements of the original materials from the approved hull plans.

Cap all through hulls not in use

Properly secure port fuel tank

Provide closure devices for engine room ventilation on Port and STBD sides. Devices must be readily available and mounted in the vicinity of the vents. Temporary means provided (foam cutouts).

Recovery ladder for Man overboard shall be provided with means to prevent shifting and grab handles. Ladder shall have means to decrease angle of vertical position for crew safety.

Provide documentation of Drug Testing Consortium for all crew to include background checks.

For the purpose of flexibility only, flexible hose installed in lengths of not more than 760 millimeters (30 inches) and subject to pressures of not more than 35 kPa (5 psig). Vessel replaced all original hard lines with all flexible lines.

Overboard discharge valve seized. All valves must be operational.

Through hull penetrations must be grounded. No grounding provided for through hulls in the lazaret(rudder, raw water)

High water alarms and automatic bilge pumps are not audible and visual at the operating station. Port side bilge pump not connected.

Through hull penetrations between engine space and lazaret are not watertight or vapor tight.

Oil pump must be hard wired. Alligator clamps are not authorized.

All cable and wire must be UL boat or marine grade wire. No dead end wires are permitted. Ensure all electrical installations are made in accordance with either 46 CFR 183 or ABYC.

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray.

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage.

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel

A hatch exposed to the weather must be watertight. Engine room hatch missing latch on one end.

Post all pages of the COI and Stability letter.

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Waterlight must be connected by a lanyard via a detachable clip.

Life floats need to be outfitted IAW 46 CFR 160.027. Arrange the painters, pendanats, paddles, waterlights IAW the e above. Life float must be international orange.

Post life jacket donning placards for all types of life jackets on board.

Post SPV decal.

Vessel must have life jackets on board for all passengers. 01 child life jacket condemned. Replace SOLAS tape on 01 adult life jacket.

Must have either 6 red and 6 orange flares or 12 red flares. Vessel missing 4 flares.

EPIRB must be located clear of all obstructions. Mount in a float free location.

Provide light for magnetic compass.

Vessel VHF radio must have GPS input provided.

Vessel must have single side band radio installed and operational per FCC requirements. Route restricted to 20NM.

Vessel all around white light not operational.

Conduct underway drills with Marine Inspectors in attendance.

Demonstrate proper operation of pre-engineered fire extinguishing system.

FWD bilge high level alarm visual indicator is extinguished. Repair/renew.

All strobes have incorrect batteries, and will not float. Renew batteries and prove proper operation.

BII fire extinguisher is not mounted in the correct bracket and will not stay in place. Provide and mount correct bracket.

Compass light is extinguished. Repair/renew.

All nautical charts and publications are out of dat. Renew.

First aid kit is missing contents. Renew contents per CFR.

Positive cable from battery to starter is smashed and has missing insulation. Replace/repair.

RACOR filter is missing a flame bowl repair/replace.

Life float reflective tape is wasted and must be replaced.

Life ring is incorrect size. Provide 24" life ring.

Vessel was missing 1 B-II fire extinguisher.

Vessel did not have any 2.5 gallon fire buckets.

Vessel had expired alcohol test strips.
