PSCO observed missing seal on the COT#1 PRV. Additionally, the ship does not have any official documentation from the Administration or class accepting the sealing of both port and starboard deck tank pressure relief valve seals.

Gas detection equipment acceptable to the administration and suitable for the gases to be carried should be provided in accordance with column f in the table of chapter 19. During COC examination, the vessel was using 35% LEL propane to calibrate their fixed gas detection, while the vessel was loaded with anhydrous ammonia. The current arrangement was designed to reflect the flammability dangers of anhydrous ammonia but failed to capture the toxic dangers of anhydrous ammonia. The safety sheet issued to the vessel stated that the short term exposure limit was 25 ppm. 1410-Propulsion Main Engine-The condition of the ship and its equipment shall be maintained to conform with the present regulation to ensure that the ship will remain fit to proceed to sea. 1899-(other)Tankers-An airlock should only be permitted between a gas-dangerous zone on the open weather deck and a gas safe space and should consist of two steel doors substantially gas tight. 1870-Tankers-special requirements-electric motor rooms cargo compressor rooms, and pump rooms; Provision should be made to ventilate such spaces prior to entering; a warning notice requiring the use of such ventilation should be placed outside the compartment. Vessel did not have a sign outside of cargo compressor room. 74 SOLAS (14) VII/13.1, IGC (93)12.1.1.
