Pressure relief valves should be set and sealed by a competent authority acceptable to the Administration and a record of thsi action, including the valves of set pressure should be retained aboard the ship. All four tank PRVs were missing an identification tag and all four pilot operator had painted over tag that were unreadable. The sealing wire broken on three of the four pilots.

Electrical installations should be such as to minimize the risk of fire and explosion from flammable products. The power cable to tanks 1 and 2 sounding device pulled partially out of the cable gland. The connection box for tank 1 was sealed with putty and appeared not to seal as designed.

Bonding straps are required for cargo and slop tanks, piping systems and equipment which are permanently connected to the hull of the ship. IEC 92-502/5.5.3
Bonding straps on the vapour lines, starboard side near the ESD valves, were missing.
