Gas detection equipment acceptable to the administration and suitable for the gases to be carried should be provided. Fixed gas detection equipment failed calibration multiple times, sensor was replaced and calibration completed. Flow failure alarm was never demonstrated to be operational as outlined in fixed gas detection manual. Vessel to provide classification society attestation that fixed gas detection system installed is fully operational as per onboard maintenance manual. Clear prior to loading cargo.

A water spray system for cooling, fire prevention, and crew protection should be installed to cover; exposed cargo domes and any exposed parts of cargo tanks. The port manifold and the #1 port and starboard tank domes were not properly protected by the water spray system.

Intrinsically safe electrical equipment and wiring may be fitted in all gas-dangerous spaces and zones. There was exposed dead end wiring in the motor room and a non-intrinsically safe light fixture above the #3 tank top. Rectify prior to loading cargo.
