No extensive alterations involving the safety of a tank vessel either in regard to hull or machinery shall be made without the approval of the Commandant. Before such alterations are carried out, copies of plans and specifications in triplicate for the work involved shall be forwarded to the Officer in Charge, Marine Inspections. Two 5,000 gallon potable water tanks are mounted on wooden blocks on board tank barge, with no documentation of approval from USCG. Vessel to submit plans of modifications to MSC for approval within 7 days and provide proof of submission to Sector New Orleans. Vessel to include all changes to machinery, structure, outfitting, and equipment that affect the lightship parameters referenced in MSC approval modifications letter C1-1604492, dated December 29, 2016. Deficiency considered rectified upon receipt of MSC approval letter. 46 CFR 31.10-25

Each vessel vapor connection flange face must have a permanent stud projecting outward....and is at least 1 inch long. it must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. The stud on the vapor line was pushed in, and in the wrong location. Replace stud and spool piece prior to cargo operations requiring use of the VECS system.46CFR39.2001(j)
