EACH CARGO TANK OF A TANK BARGE MUST HAVE ONE OF THE FOLLOWING LIQUID OVERFILL PROTECTION ARRANGEMENTS: A SYSTEM MEETING THE REQUIREMENTS OF 46 CFR 39.2007, HIGH LEVEL AND TANK OVERFILL ALARMS. MI WAS UNABLE TO TEST ANY OF THE ALARMS DUE TO A MALFUNCTION OF THE SYSTEM. 46 CFR 39.2009(a)(1)

No person may use any equipment listed in paragraph (c) of this section for transfer operations unless the vessel or facility operator, as appropriate, tests and inspects the equipment. The frequency of the tests and inspections required by this section must be: Each vapor hose, vapor collection arm, pressure or vacuum relief valve, and pressure sensor is tested and inspected for vessels annually, or as part of the biennial and mid-period inspections. MI observed no available valid records or indication of cargo system pressure vacuum relief valve testing.

On a tank barge, each cargo tank must be equipped with an overfill device that meets the requirements of 46 CFR 39.2009 (a)(1). MI unable to witness operational testing of audible and visual alarm indicators of high/overfill alarms.

Vapor piping systems and pressure vacuum valves that are used for polymerizing cargoes must be inspected internally at least annually. MI unable to safely inspect vapor piping during annual inspection as both end plates were not removed.

No extensive alterations involving the safety of a tank vessel either in regard to hull or machinery shall be made without the approval of the Commandant. Before such alterations are carried out, copies of plans and specifications in triplicate for the work involved shall be forwarded to the Officer in Charge, Marine Inspections. Two 5,000 gallon potable water tanks are mounted on wooden blocks on board tank barge, with no documentation of approval from USCG. Vessel to submit plans of modifications to MSC for approval within 7 days and provide proof of submission to Sector New Orleans. Vessel to include all changes to machinery, structure, outfitting, and equipment that affect the lightship parameters referenced in MSC approval modifications letter C1-1604492, dated December 29, 2016. Deficiency considered rectified upon receipt of MSC approval letter. 46 CFR 31.10-25

Each vessel vapor connection flange face must have a permanent stud projecting outward....and is at least 1 inch long. it must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. The stud on the vapor line was pushed in, and in the wrong location. Replace stud and spool piece prior to cargo operations requiring use of the VECS system.46CFR39.2001(j)

The last 1 meter of vapor piping upstream of the vessel vapor connections and each end of the vapor hose must be marked in accordance with this cite. There were no markings on the vapor pipeline. Paint and stencil the appropriate markings prior to cargo operations requiring use of the VECS system. 46CFR39.2001(h)

A pressure gage shall be installed at each pump discharge, and it shall be located at a point visible with respect to the pump controls. Pressure gauge on cargo pipe is broken and illegible. Replace gauge prior to cargo operations.46CFR32.50-5

Fuel piping between on-deck diesel holding tank and prime mover has a leak that is patched with epoxy.

Wiring Harness on Prime Mover is not intrinsically safe, explosion proof, or purged and pressurized for a Class I Div I area. Ref D8(dp) Policy Letter 02-2007 CH1. Barge is allowed to operate until Aug 08 per policy ltr.
