11 - Life Saving Appliances › N/A - No Subsystem › Rescue boats

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use [20.2]. Rescue boats should be stowed in a continuous state of readiness for launching in not more than five minutes [14.1]. At approximately 1145 local time, PSCO asked to witness operational test of rescue boat engine. Crew could only start rescue boat using ether and rescue boat engine would not run for more than 5 seconds after start. At 1430 local time, rescue boat engine remained inoperable. PSCO observed crew rebuilding the carburetor and replacing the spark plugs in an attempt to bring engine to operational status. 74 SOLAS (14) III/20.2 74 SOLAS (14) III/14.1 30abc

The Company should ensure that inspections are held at appropriate intervals, any non-conformity is reported, and appropriate corrective action is taken. Company SMS manual, Chapter 9.4 requires "all employees and everyone involved in Company's activities should report any deficiency or hazard. The report may be verbal or in writing, in which case the form NCR 001...may be used." There is no objective evidence of non-conformities reported via form NCR 001, nor any other objective evidence of appropriate corrective action taken, for the operational/technical deficiencies #1 and #3-#7. Company SMS requires Form M-028b, "Planned Maintenance Program for Bridge Equipment, Life Saving Appliances and Firefighting Equipment", to be completed each month, and to "tick with the appropriate marks for each box next to the maintenance routines". Three different marks are provided for OK ([check-mark]), NOT APPLICABLE (N/A), and NOT OK (NOT OKAY). The May and June forms have "X" marks only and has zero items marked as "not ok", even though there are multiple items listed (ex: low pressure CO2, fixed dry powder) with which the vessel is not equipped. **An additional external verification of the ISM-SMC by the RO is required. 74 SOLAS (14) IX/3.1 REF ISM Code/10.2 30abc

Resolution: Received Lloyd's Register SMS Audit results and notes which addressed all ISM items listed in the deficiency and corrective and preventative measures to ensure compliance. LR recommends a future ISM audit within three months for follow ups.

The following structural fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions [II-2/14.2.1.1]. When a fixed gas fire-extinguishing system is used, openings which may admit air to, or allow gas to escape from, a protected space shall be capable of being closed from outside the protected space [II-2/10.4.2]. Doors to purifier room are "A" class doors per vessel's Fire Control Plan. Aft "A" class door frame found to be holed, compromising the integrity of the "A" class division. Additionally, purifier room is protected by fixed CO2 fire-extinguishing system capable of being actuated separately from main engine room fixed CO2 system.
