No OSV may carry flammable or combustible liquid cargoes in bulk without approval. In accordance with the approval letter for the vessels methanol arrangements(E1-0504909) dated 17JUN2005, the methanol P/S/C tanks including the associated fittings shall comply with MVI Policy Letter No. 91-93, dated 30SEPT1993, "Carriage of Methanol (Methyl Alcohol) on Offshore Supply Vessels". This required offshore supply vessels to meet the venting requirements of 46 CFR 32.55, which states that cargo tanks in which Grade B or C liquids are to be transported shall be fitted with either individual pressure-vacuum relief valves which shall extend to a reasonable height above the weather deck or shall be fitted with a venting system consisting of branch vent lines connected to a vent header which shall extend to a reasonable height above the weather deck and be fitted with a flame arrester or a pressure-vacuum relief valve. Vessel's venting terminated in a deck containment box which inhibits the ability for the proper venting of the methanol tanks. Electrical equipment should not be installed in hazardous locations unless essential for operational purposes. When installed in these locations, special precautions should be taken to ensure that the electrical equipment is not a source of ignition. Pump exhibited extensive oxidation and presented a possible ignition source. All tanks carrying methanol shall have either a restricted or closed gauging device. Vessel had only open gauging arrangements. Vessel must complete a satisfactory inspection of the Methanol transfer system/equipment to include: Material condition of the transfer pump, associated pressure gauge, manual shutoff valve, armored cabling/remote shutdown, installation and approval of authorized venting arrangements, and proof of test documentation for venting arrangements, and installation of authorized gauging arrangements prior to the carriage of methanol. Vessels assigned loadlines must have loadline marking and deck-line marking permanently scribed or embossed. Loadline marking on the starboard side of the vessel found to be painted ineligibly and in need of re-marking.

Vessels less than 100 gross tons and equipped with an independent auxiliary means of steering must have a fixed means of communication between the pilothouse and the auxiliary operating station. Sound-powered phone at the forward operating station in the pilothouse found to be inoperable; unable to communicate with the auxiliary operating station within the engine room.

Vessel owners or employers shall provide clear and conspicuous signs on board notifying the crew of the presence of video and audio surveillance equipment. Vessel was found to not have any postings notifying the crew of the presence of video and audio surveillance equipment. 46 USC 4904

The inspection for certification is made by a marine inspector to ensure the vsl is safe including; main machinery and fire hazards. The MDE (port and stbd) fuel hoses between the primary and secondary filters are deteriorated beyond repair. Replace hoses.

Each battery in an item of survival equipment not marked with an expiration date must be replaced annually. Float light batteries need to be replaced.

Vessel must undergo an annual inspection within 3 months before or after each anniversary date. Annual was completed 14 days past its 3 month anniversary date and continued to operate. 46 CFR 126.510(a)

Vessels subject to initial and periodic Load Line surveys shall have annual surveys, within 3 months either way of the certificate's anniversary date. Vessel's Load Line survey was completed two days past its 3 month anniversary date of March 14,2021. 46 CFR 42.09-15(d)

At least one abandon-ship drill each 3 months must be held at night, unless the master determines it unsafe. Vessel was completing drills at required intervals, but none completed at night. 46 CFR 131.530(b)(9)

A vessel of less than 100 gross tons must have adequate emergency lighting fitted along the line of escape to the main deck from accommodations and working (machinery) spaces below the main deck. 03 E-lights in engine room were inoperable. Switchboard, center aisle, and escape trunk WTD would not energize. 46 CFR 129.440(a)

There must be at least two means of escape from a space where crew may be employed. Engine room escape trunk deck hatch spring assist needed adjustment to allow easier opening. 46 CFR 127.240(a)(2)

An emergency means of communication must provide common talking means of two-way voice communication and calling among the navigating bridge and alternative steering station. Sound powered phone between bridge and control station in engine room was inoperable. 46 CFR 113.30-5(a)(3)

In a space which the voice communication station audible-signal device cannot be heard, there must be another audible-signal device or a visual-device, such as a light. Engineer's assist blue light in engine room was inoperable. 46 CFR 113.30.25(g)

Piping system designs shall account for the effects of ship motion and flexure, including weight, yaw, sway, roll, pitch, heave, and vibration. Port generator prime mover return fuel flexible hose had excessive vibration when running. 46 CFR 56.07-10(c)

Power operated valve actuators installed in liquid mud must be capable of closing the valves under all conditions,except during physical interruption of the power system. Cargo valves located at each liquid mud tank were being repaired and not operable. 46 CFR 56.50-60(d)(3)(i)

A vessel security plan that is approved by the MSC is valid for 5 years from the date of its approval. Vessel's security plan was expired and needs to be resubmitted to be valid in accordance with 33CFR104.410(f).

Master or person in charge shall ensure that the ship is not operated unless a garbage management plan is on the ship and that each person handling garbage follows the plan. Vessel did not have a waste management plan on board. Waste management plan needs to be created in accordance with 33CFR151.57(a)

A vessel of 12 meters or more in length shall be provided with a whistle. Whistle was not operable at time of inspection. Make repairs in accordance with 33CFR83.33(a)

Vessels subject to Vessel General Permit (VGP) who are required to submit a Notice of Intent (NOI) must submit an annual report each year. Vessel has not submitted an annual report since 2016. 40 CFR 122 FINAL 2013 VGP 4.4.1

Nonmetallic flexible hose must be in accordance with SAE J1942. Hydraulic bowthruster hose was severly chaffed. 46 CFR 56.60-25(b)(1)

No auxiliary steering need be installed if the main steering gear is installed in duplicate. When conducting steering test during inspection, inspector noticed low level alarm for the hydraulic oil tank on pump #1. During examination of pump, noticed substantial leak coming from hydraulic filter and improper tank level sensor. Owner shall correctly install filter and sensor, and provide evidence to MI of satisfactory operation.

A rudder-angle indicator independent of the control of the main steering gear must be installed at the steering-control station in the pilothouse. While conducting steering checks on the forward station at the bridge, MI noted 10 degree variance between port and starboard rudder-angle indicators. Owner shall provide evidence of adjustment to starboard rudder-angle indicator to correctly display rudder position.

Each vessel must be operated in compliance with 33 CFR 155. Vessels carrying oil as secondary cargo must carry appropriate equipment and supplies for the containment and removal of on-deck oil cargo spills of at least one half barrel. The equipment is listed in 33 CFR 155.220(b). The owner shall provide evidence of spill equipment prior to carriage of oil as secondary cargo.

All systems for detecting smoke and fire, including sensors and alarms, must be inspected and tested. Smoke detector in galley was not operational. Owner shall replace the detector and provide evidence of functioning detector in galley space.

isible and audible alarms must be installed at the pilothouse to indicate the following IAW 46 CFR 130.460. The panels installed on the forward and aft stations on the bridge were faded and difficult for the captain to read. Owner shall provide evidence of installment to all ow for easier viewing or replacement of screens on alarm panels.

Each lifefloat on board must have two paddles that are at least 4' long and buoyant. The paddles (x4) for each lifefloat (x2) were cracked and not serviceable. Owner shall replace the paddles and provide evidence of suitable ones on board and lashed to the lifefloats.

Enginge Room bilge float switch did not function. Owner shall provide evidence of bilge high level alarm working.

Local alarm panel in engine room was defective and unreadable. Owner shall replace panel with satisfactory display.

Provide embarkation lighting at life raft embarkation station.

Remove temporary wiring installed as hard wiring at junction box on bridge.
