The owner or operator of an OSV shall make the vessel available for inspection by a marine inspector each time an accident occurs, or a defect is discovered that affects the safety of the vessel. Vessel was discovered in drydock at Steiner Shipyard with starboard rudder missing and crack in starboard propeller. Conduct repairs in accordance with 46 CFR 126.150 with oversight by a Coast Guard Marine Inspector.

Each lifefloat must be approved under approval series 160.027 and be equipped. Netting deteriorated and ripped in several areas on six person liferaft. Restricted to 36 persons allowed onboard until replaced.

Each generator and motor, except a submersible-pump motor, must be in an accessible space, adequately ventilated and as dry as practicable. Port and STBD generator windings and stators require cleaning.

Each vessel must be designed, constructed, and maintained to minimize fir hazards, as far as reasonable and practical. The started on the port propulsion engine and the bow thruster engine blew atomized fuel out during the starting process.

Except in an emergency, no repairs or alterations to the hull or machinery, or to equipment that affects the safety of the OSV, may be made without notice to the cognizant OCMI in the inspection zone where the repairs or alterations are to be made. When the repairs or alterations have been made, notice must be given to this OCMI as soon as practicable. Prior to annual inspection vessel noticed bow thruster reservoir was not holding water. Vessel determined leak was coming from external keel cooler. Vessel could not be placed on drydock in AOR and requested to shift to Berry Brothers yard for drydock space. Vessel must conduct repairs at Berry Brothers yard and contact MSU Morgan City for approval of repairs.

All waterlight batteries were expired. Vsl to replace all batteries for waterlights.

Drill not conducted due to crew not onboard. Conduct drills in presence of Marine Inspector prior to operation.
