The owner or operator of an OSV shall make the vessel available for inspection by a marine inspector each time an accident occurs, or a defect is discovered that affects the safety of the vessel. Vessel was discovered in drydock at Steiner Shipyard with starboard rudder missing and crack in starboard propeller. Conduct repairs in accordance with 46 CFR 126.150 with oversight by a Coast Guard Marine Inspector.

Resolution: Attended vessel to verify installation of new prop and rudder. Also received objective evidence.

Each lifefloat must be approved under approval series 160.027 and be equipped. Netting deteriorated and ripped in several areas on six person liferaft. Restricted to 36 persons allowed onboard until replaced.

Except in an emergency, no repairs or alterations to the hull or machinery, or to equipment that affects the safety of the OSV, may be made without notice to the cognizant OCMI in the inspection zone where the repairs or alterations are to be made. When the repairs or alterations have been made, notice must be given to this OCMI as soon as practicable. Prior to annual inspection vessel noticed bow thruster reservoir was not holding water. Vessel determined leak was coming from external keel cooler. Vessel could not be placed on drydock in AOR and requested to shift to Berry Brothers yard for drydock space. Vessel must conduct repairs at Berry Brothers yard and contact MSU Morgan City for approval of repairs.
Action required: 15 - Rectify deficiencies by next port
