To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. The valve must be operable from a safe place outside the space where the valve is installed. The Fuel Shut offs for the engines were not able to be actuated. Exercise and test fuel shut offs for both engines and generators to the satisfaction of the TPO. Provide objective evidence to the CG.
Action required: 17 - Rectify deficiencies prior to departure

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. Port main propulsion engine was producing excessive output in the crankcase ventilation outlet leading to a fire on the engine.
Provide causal analysis and technical report once port main propulsion engine repair is complete to include satisfactory operational tests have been completed, and attesting vessel is fit for intended route and service.
Action required: 60 - Rectify deficiencies prior to movement
