All installations must be of marine type engines suitable for the intended service...During inspection of the port side cargo pump, observed visible exhaust leak and burnt lagging on the exhaust piping near the turbocharger. 46 CFR 32.35-1 46 CFR 58.10-5(a)

The frame or support of each size III,IV,and V fire extinguisher required by table 34.50-10(a) must be welded or otherwise permanently attached to a bulkhead or deck. Vessel did not have its cargo tank area semi-portable fire extinguisher permanently attached to a bulkhead or deck.

The high-level alarm required by paragraph (a) of this section must - (2) Be identified with the legend “High-level Alarm” in black letters at least 50.8 millimeters (2 inches) high on a white background. The tank overfill alarm required by paragraph (a) of this section must - (2) Be identified with the legend “TANK OVERFILL ALARM” in black letters at least 50.8 millimeters (2 inches) high on a white background. Vessel did not have adequate labeling of the high level and overfill alarms.

If the periodic inspection reveals deficiencies in your vessel's maintenance, you must make any or all repairs or improvements within the time period specified by the OCMI. Observed missing gasket in the starboard forepeak hatch. 46 CFR 31.10 17(b)(3)

A pressure gage shall be installed for each pump discharge, and it shall be located at a point visible with respect to the pump controls. Observed malfunctioning pressure gauge on the port cargo pump. Replace prior to use of port cargo pump. 46 CFR 32.50 5(a)

Each cargo tank of a tank barge must have one of the following liquid overfill protection arrangements. Observed inoperable loss of power alarm due to dead batteries. Replace prior to loading cargo operations. 46 CFR 39.2009(a)

Each cable installation must meet - sections 25, except 25.11, of IEEE 45-200. Observed multiple broken/missing band clamps for wire cables under the port and stbd house. Install clamps every two feet or every frame, whichever is less. 46 CFR 111.60 5(a)(1)

Pumprooms on tank vessels carrying Grade A, B, C, D and/or E liquid cargo - TB/ALL. (a) Cargo pumps. In tank vessels carrying Grade A, B, C, or D liquid cargo, cargo pumps shall be isolated from source of vapor ignition by gastight bulkheads. A gastight bulkhead between the pumproom and the pump engine room may be pierced for drive shaft and pump engine control rods provided such openings are fitted with stuffing boxes or other approved gland arrangement. Both port and starboard cargo pump room shaft vapor seals were deteriorated, worn, or torn to the point that vapor tightness was unable to be maintained. Vessel shall not conduct cargo operations until suitable replacement are in place and vapor tightness is resumed.

Gears, couplings, flywheels and all machinery capable of injuring personnel shall be provided with adequate cover or guards. Generator had no protective cover for the flywheel belt.

Subpart B-Required Chemical Testing Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49CFR Part 40. Vessel representative could not produce and records of required chemical testing, tracking, or sampling.

Nonmetalic flexible hose may be used in lube oil, fuel oil, and fluid power systems only where flexability is required and in lengths not exceeding 30 inches. 46 CFR 56.60-25(b)4

Each vessel must carry lifejackets for each person onboard. Each lifejacket must be marked with type 1 retro-reflective materials. Both lifejackets are not type 1 and have no retro. Replace to the satisfaction of the attending CG inspector. 46 CFR 199.70(b)

Each vessel vapor connection flange face must have a permanent stud projecting outward, loacated at the top of the flange. Both manifold vapor flanges did not have a stud intalled. 46 CFR 39.2001(j)

The frame or support of each size III, IV, and V fire extinguisher required by table 34.50-10(a) must be welded or otherwise permanently attached to a bulkhead or deck. Semi-portable extinguisher was not secured to the vessel. 46 CFR 199.290(c)

Each lifesaving appliance must be in working order. Both life rings were found with deteriorated grab lines. 46 CFR 199.190(a)

Identified areas of hull plating to be cropped and renewed; 1 on port side sheer strake amidships, 1 on starboard quarter at the turn of the notch. See 835 for details.

Pressure guage shall be installed for each pump discharge, and it shall be located at a point visible with respect to the pump controls. Gauge is not visible from operating system.
