A whistle fitted in a vessel less than 20 meters in length shall provide an audible range of not less than 0.5 nautical miles. Vessel's installed sound producing device was inoperable. 33 CFR 86.01(c).

The master shall conduct sufficient drills and give sufficient instruction to make sure that all crewmembers are familiar with their duties during emergencies that necessitate abandon ship or the recovery of persons who have fallen overboard. Abandon ship drills and training shall be logged or otherwise documented. The drill entry shall include the date of the drill and training and the general description of the drill scenario and training topics. The vessels master could not provide logs or evidence of man overboard or abandon ship drills or training. Additionally, the vessels crew was unfamiliar with their duties during man overboard drill and did not receive training onboard the vessel. Vessel shall conduct man overboard drills and training prior to getting underway to the satisfaction of the Coast Guard. 46 CFR 185.520(a) & (f)

The owner or master shall instruct each crewmember, upon first being employed for the first time on a particular vessel and at least once every three months as to the duties the crewmember is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard. Crew training shall be logged or otherwise documented. The training entry shall include the date of the training and the general description of the training topics. The vessels owner or master could not provide logs or evidence of training. Vessel shall conduct training prior to getting underway to the satisfaction of the Coast Guard. 46 CFR 185.420(a) & (c)

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines, one at the tank connection and one at the end of the fuel line to stop fuel flow when servicing accessories. During inspection it was discovered that the forward fuel tank emergency remote fuel shutoff valve was located at a secondary valve down line from the fuel tank, approximately 2' from the valve installed at the fuel tank. At next drydock exam vessel shall alter placement of forward emergency fuel shut off valve to be located at the closest valve to the fuel tank. 46 CFR 182.20-40(b)(3)

Each ring life buoy on a vessel must be stowed in a way that it can be rapidly cast loose. During the man over board drill, it was discovered that the attached life line was tied up in a way which prevented it from being rapidly cast loose. The deckhand made two unsuccessful attempts to deploy the ring buoy prior to securing the drill. Vessel shall ensure all lifelines onboard the vessel are stowed in a way which allows rapid cast prior to departure. 46 CFR 180.70(b)(3)

Battery is missing a vent cap which needs to be replaced. Batter has excessive corrosion on one terminal which needs to be cleaned. Battery container has some standing oily water in it which needs to be removed.

MDE fuel return line has a small section that is chafed beyond the outer cover and needs to be replaced.
