AFTER ANY SURVEY OF THE SHIP UNDER REGULATIONS 7,8,9,OR 10 HAS BEEN COMPLETED, NO CHANGE SHALL BE MADE IN THE STRUCTURAL ARRANGEMENTS, MACHINERY, EQUIPMENT AND OTHER ITEMS COVERED BY THE SURVEY, WITHOUT THE SANCTION OF THE ADMINISTRATION. PSCO RECEIVED REPORT FROM THE VESSEL THAT THERE WAS A BLACKOUT INCIDENT DUE TO POWER FAILURE OF ALL THREE AUXILIARY ENGINES DUE TO A JET SYSTEM FAILURE, WHICH AFTER TROUBLE SHOOTING WAS DETERMINED TO BE WATER IN THE MGO FUEL LINE FROM THE STARBOARD SERVICE TANK (#1 ALP STORAGE TANK). VESSEL CONDUCTED REPAIR BY BLANKING OFF AND REMOVED THE STEAM LINE OF THE #1ALP STOARGE TANK.
Action required: 15 - Rectify deficiencies by next port
