Fire fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed emergency fire pump not able to produce sufficient pressure.

Rescue boats shall be stowed in a state of continuous readiness for launching in not more than 5 minutes. PSCO observed rescue boat unable to start in the allotted time.

General arrangement plans shall be permanently exhibited for the guidance of the ship's officers, the various fire sections enclosed by "A" Class divitions. PSCO observed self-closing "A" Class doors not latching shut.

If a wrong entry has been recorded in the Oil Recorded Book (ORB) Part 1, it should immediately be struck through with a single line in such a way that the wrong entry is still legible. The wrong entry should be signed and dated, with the new corrected entry following. Vessels ORB had white out marks on multiple occasions in the last 03 months. The safety management system should ensure: compliance with mandatory rules and regulations. The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of relevant rules, regulations, codes, and guidelines. The operational nature of deficiencies 1, 2, 3, and 6 provide objective evidence of a serious failure of the implementation of the ISM Code. An external audit report satisfactory to the OCMI must be submitted prior to clearance of the deficiency. ISM Code (2018) 1.2.3.1 and 6.4

The purpose of this regulation is to provide means of escape so that persons on board can safely and swiftly escape to the lifeboat and liferaft embarkation deck. For this purpose, the following functional requirements shall be met. Escape route shall be maintained in a safe condition, and clear of obstacles. There was a padlock present on the outside aft B deck weathertight door. The lock did not have means of removing it from the inside in the event of an emergency, blocking means of escape from the B deck. Many of the other weathertight doors were observed with a means to lock the weathertight door in the same fashion with no lock present. Referencing ISPS Code Part B 8.10.1, the Ship Security Assessment should take into account all possible vulnerabilities, which may include conflicts between safety and security measures. SOLAS 74 (2020 Cons) II-2 Reg 13.2

THE CONDITON OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO CONFORM WITH THE PROVISIONS OF THE PRESENT REGULATIONS. PSCO FOUND PORTSIDE SCUPPER INCAPABLE OF BEING SEALED, CAUSING OILY WATER TO BE DISCHARGED.

NOT LESS THAN ONE HALF OF THE TOTAL NUMBER OF LIFEBUOYS SHALL BE PROVIDED WITH LIFEBUOY SELF IGNITING LIGHTS. IT WAS DISCOVERED BY PSCO'S THAT LESS THAN HALF OF THE VESSEL'S LIFEBUOYS WERE PROVIDED WITH LIFEBUOY SELF EGINITING LIGHTS. LIGHT WAS REMOVED FROM PILOT EMBARKATION BACK TO THE LOCATION WHERE IT WAS INTENDED LEAVING PILOT EMBARKATION LIFEBUOY WITHOUT A SELF IGNITING LIGHT.

0635- Life Saving Appliances- Recovery time of the rescue boat shall not be more than 5 minutes in moderate sea conditions when loaded with its full complement of persons and equipment winch for rescue boat davit missing. Vessel is unable to recover rescue boat in less than 5 minutes.-74SOLAS(2009 Cons.) Chapter III/17- 17(a)(c)

Charge air cooler inoperable.

The vessel's echo depth sounder was not fitted with a device to continously record soundings. The equipment was new and had a printer option available, but no printer was connected to the device.

The vessel did not maintain a logbook for medicine and/or narcotic distribution.
