Reinspection: All other vessels must undergo an inspection for certification as specified in § 176.404 and annual inspection as specified in paragraph (b)(1) of this section.(1) Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Annual inspection was due NLT than 5/21/2025. Vessel is restricted from carrying passengers until completion of annual COI inspection.

Each ring life buoy on a vessel must:(1) Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150; Ring life buoys grab lines were completely wasted. Replace life buoy and provide evidence to the attending marine inspector.

For each marine casualty required to be reported by § 185.202, the owner, agent, master, or person in charge of the vessel shall determine whether there is any evidence of alcohol or drug use by individuals directly involved in the casualty. Alcohol test strips were expired. Provide valid alcohol test strips to the marine inspector.

46 CFR 185.520(f) - Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Provide log for drills and inspections.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Analgesic medication was expired. Provide valid medication to the attending marine inspector.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel's alcohol test strips were expired. Demonstrate that the vessel has access to alcohol test strips within two hours of a serious marine incident.

Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. Vessel could provide valid certificate of documentation. Provide valid certificate of documentation for FY24 to the attending marine inspector.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. Vessel charts we from 2016 and did not have current navigation publications. Provide the following listed below to the satisfaction of the attending marine inspector. (1) Updated navigational charts (2) U.S. Coast Pilot or similar publication (3) Coast Guard Light List (4) Tide Tables (5) Current Tables

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. Bow thruster did not shutdown when the fixed fire extinguishing system was tested. Found the air solenoid pin removed from the shutdown unit on the bow thruster engine. Vessel replaced the pin with a test SAT.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Sluice valves are not permitted in watertight bulkheads. Vessel's bilge manifold system was unable to get suction to drain the Tank Space, Engine Space or Lazarette. Demonstrate the ability to pump all watertight compartments to the satisfaction of the attending USCG marine inspector.

A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. Vessel's licensed Master did not have medical certificate available for inspection. Provide proof of medical certificate or correspondence from NMC attesting to valid medical certificate.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel did not have access to alcohol testing devices. Demonstrate that vessel has access to alcohol testing devices for crew within 2 hours of a serious marine incident.

The master of a vessel, except a vessel listed in § 185.502(a) of this part, shall keep a correct, written count of all passengers that embark on and disembark from the vessel. Prior to departing on a voyage, the passenger count must be communicated verbally or in writing, and available ashore at the vessel's normal berthing location or with a representative of the owner or managing operator of the vessel. The passenger count shall be available to the Coast Guard upon request. Vessel does not have a system for a passenger count available ashore.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First aid kit contained expired aspirin. Provide complete and current first aid kit.

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. Life ring water light battery found expired, installed in 2013. manufacturer instructions require changing annually.

The pump outlet must be fitted with a pressure gauge. No gauge was fitted. Provide gauge.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Engine space ventilation dampers did not close completely when tested.

Electrical equipment on a vessel must be installed and maintained to: (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; (c) Minimize accidental personnel contact with energized parts. Electrical receptacle in the space below the galley found without a cover plate, and receptacles were found partially installed on an energized circuit missing cover plates and with cable ends terminated using twist on connectors. Replace missing covers and complete installation. De-energize circuit until completed.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Records for abandon ship and man overboard drills were not available. Conduct drills and provide records.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First aid kit was not compelte or current. Provide complete and current first aid kit.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child life jackets were not seperate from adult. Seperate child jackets.

A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided, or such greater number as necessary to provide a life jacket for each person being carried that is smaller than the lower size limit of the adult life jackets provided to meet this section, except that: When all “extended size” life preservers (those with a lower size limit for persons of 1,195 millimeters (47 inches) in height or weighing 20.4 kilograms (45 pounds)) are carried on board, a minimum of only 5% additional child size devices need be carried. Vessel was missing one approved type 1 child sized life jacket. Provide additional jacket. Until provided, carriage is limtied to eight (8) persons under 90 pounds.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18) (incorporated by reference; see 46 CFR 175.600). Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. Lower deck life jacket stowage was not marked with number of life jackets.

REPLACE LIFE RING BUOY

PROVIDE SPANNER WRENCHES FOR ALL FIRE STATIONS.

REPAIR FIRE PUMP SEAL.

REPAIR/REPLACE RPM GAUGES ON BRIDGE.

Demonstrate proper operation of port side running light. Vessel restricted to daylight operations only until corrected.

Provide current charts, light list, coast pilot and currents/tides.

Provide (09) child sized lifejackets. (08) were found on board.

Rudder stops are hitting the midship bulkhead. Repair bulkhead and adjust rudder stops to prevent recurrence.

Prove proper operation of navigation lights prior to operating at night.

Install blanks on electrical panels in galley and E/R.

Update drill logbook to include MOB, FIRE, and ABANDON SHIP drills every 3 months.

Mark aft emergency escape hatch and locate hatch key on second deck.

Provide B-I fire extinguisher for bridge.

Properly mount main deck and upper deck bar area portable fire extinguishers.

Laz: Install bracket for nitrogen bottle for fire extinguishing system.

Prove proper operation of nav lights.
