EACH SHIP SHALL HAVE A BALLAST WATER RECORD BOOK WHICH SHALL AT LEAST CONTAIN THE IN FORMATION SPECIFIED IN APPENDIX II. PSCO OBSERVED ALARMS AND TRIPS FROM BALLAST WATER MANAGEMENT SYSTEM NOT BEING RECORDED IN BALLAST WATER MANAGEMENT SYSTEM RECORD BOOK.

Before the ships leave port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. PSCO observed 03 self igniting lights not operational.74 SOLAS (20) III/20.2

The company and the ship shall comply with the requirements of the International Safety Mangement Code. For the purpose of this regulation, the requirements of the code shall be treated as mandatory. The company should establish procedures to ensure the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. PSCO identified that vessels maintenance procedures do not provide sufficient instructions on how to properly conduct maintenance on life buoy self lgniting lights. Manufactuers instructions are not provided in the vessels SMS for any of the self igniting lights currently onboard. Conduct internal safety audit within 90 days. 74 SOLAS (20) IX/3.1 ISM Code 10.1.

To assist in casualty investigations, ships, when engaged on international voyages, subject to the provisions of regulation 1.4, shall be fitted with a voyage data recorder (VDR). PSCO observed inoperable VDR. Vessels primary means of ECDIS is working as designed with VDR. Backup ECDIS relay input is inoperable. Repairs to be completed by Sept. 12,2022.

11101- Free-fall lifeboats shall be fitted with a safety harness at each seat in contrasting colors. PSCO found the free fall life boat has 04 safety harnesses installed that are not contrasting color. 74 SOLAS (14) III/31.1.2

14703- Remove fouling organisms from the vessel's hull, piping, and tanks on a regular basis and dispose of any removed substances in accordance with local, State and federal regulation. PSCO observed ships hull with excessive growth on hull. 33CFR 151.2050(f)

Reissued deficiency. The onboard marine sanitation device (MSD) is not labeled as required by 33 CFR 159.7(a)(1). Provide proof of manufacturers lable to the satisfaction of the USCG.

Vessel unable to produce standard discharge connection with 6 (six) hole flange. IOPP certificate form A indicates that the vessel is fitted with a standard discharge connection. There is no on board evidence that the required equipment is on board, in violation of the IOPP certificate. USCG to attest to proper equipment on board. MARPOL 73/78 Annex I/13

74SOLAS(2004Cons.) III/20.2 The Emergency Escape Breathing Apparatus (EEBD) located on the tank top level of the engine room forward, as indicated on the fire control plan and marked by IMO placard at the stowage location, is not on station. Provide EEBD prior to departure to the satisfaction of the USCG.

74SOLAS(2004Cons)II-1/26.1 Excessive accumulation of hydraulic oil on the deck in the hydraulic pump room, forward port side, do in large to insufficient coaming around hydraulic tank and pumps, presenting a significant slip, trip, and fall hazard. Clean the deck and eliminate this hazard to the satisfaction of class.

74SOLAS(2004Cons)I/II There are cracked welds for the additional brackets on both the port and starboard hausepipes (anchors) make repairs to the satisfaction of class.

74SOLAS(2004Cons)II-1/45.1.3 Remove/replace the dead end cable for the exhaust fan in the steering gear room. The exhaust fan has been removed, however the wires/cable remains and could present risk of electrical shock if energized. Make repairs to the satisfaction of class.

74SOLAS(2004Cons)III/34 LSA Code 4.2.7.2 The 100m inflatable life raft, located forward, is missing its identification card. As such there is currently no provision for marking this life raft with name and port of registry, of the ship to which it is fitted, in such a form that the ship identification can be changed at any time without opening the container. Mark to the satisfaction of the USCG.

74SOLAS(2004Cons)III/20.10 Several stowage locations for lifesaving equipment were found to either not be marked with the required IMO symbols or were marked with the incorrect IMO symbols. Specifically life rings Main deck stbd quarter, main deck stbd bow, and the life raft (100m) stowed on bow, line port side main deck and the life ring on the port bow. Properly mark to the satisfaction of class

33CFR159.7(a)(1) The onboard marine sanitation device (MSD) is not labeled as required. Provide proof of manufactures label to the satisfaction of the USCG

The #1 Steering Gear Pump was reported broken and therefore caused a reduction in maneuverability. The vessel is required to provide a class report attesting to the operational capability of the #1 steering pump prior to departure from the port of Corpus Christi.

The vessel welded a key-lock numerical punch pad to a weather-tight door that leads from the deck to the incinerator. This work has not been approved by class and has possibly compromised the water-tight integrity of the door. The seal looks good, but the welder burned a huge hole on the inside of the door. Requiring class to approve work IAW Solas/ICLL/ and SSP. (ICLL Reg. 16)

Lifesaving Plan requires the lifebouys on the port and starboard bridgewings to have a self-igniting light attached. Must be addressed to satisfaction of class. Solas 74am III/7.3

Solas Training Manual is not ship specific. Also, vessel needs to provide in each crew mess and Recreation room, or in each crew cabin as required in Solas 74am III / 35.2 & 35.3
