A tank overflow alarm must be audible and visible in the part of the deck where the containment systems are located. Tank overflow light is not illuminating.

09209 - Where cables which are installed in hazardous areas introduce the risk of fire or explosion in the event of electrical fault in such areas, special precautions shall be taken. The gasket and encasement for the junction box on deck near the pump for cargo tank No. 4 (port side) was deteriorated preventing the enclosure from sealing causing an unsafe environment in a hazardous area. 74 SOLAS (04) II-1/45.5.4

09209 - Cables and wiring shall be installed and supported to avoid chaffing or other damage. The wiring to the No. 2 (port side) cargo tank was missing parts of its casing leaving it exposed in a hazardous area. 74 SOLAS (04) II-1/45.5.4

09209 - No electrical equipment shall be installed in any space where flammable mixtures are liable to collect, including those onboard tankers unless such equipment is appropriate to the space concerned. Two light fixtures on the catwalk forward of the manifolds were found with water inside of the case. 74 SOLAS (04) II-1/45.10.3

09209 - Cargo hoses shall have electrical continuity over their entire length including couplings and flanges and shall be earthed for removal of electrostatic charges. The bonding straps were missing on the port common line manifold and the bonding strap was broken on the 4 port manifold near the catwalk. 74 SOLAS (14) II-2/4.5.9

07126 - Means shall be provided to control the leaks of flammable liquids. PSCO observed pooled fuel oil above No. 1 and No. 2 diesel generators. 74 SOLAS (04) II-2/4.1.1

14109 - The condition of the ship and its equipment shall be maintained to conform to the provisions of the present convention. The sample line from the oily water separateor to the oil content meter was detached preventing the oil content meter from being able to read the oil content prior to overboard discharge. MARPOL Annex I/6.4.1

15109 - The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirement which may be established by the company. When PSCOs requested checklists for the overall condition of electrical equipment in the cargo area of the deck the vessel's crew stated there were no procedures in place to check the condition of electrical equipment on deck. This includes junction boxes, lighting, etc. Due to the objective evidence in the above deficiencies the vessel is not in substantial compliance with relevant conditions. The COTP questions the adequacy and overall implementation of the vessel's SMS under the ISM Code. An internal audit is required within 30 days to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the audit report to the USCG when completed.

09298 - Guard rails fitted on superstructures and freeboard decks shall have at least three courses where operationally necessary. Chains fitted between two fixed stanchions are acceptable in lieu of guard rails. PSCO observed one broken bottom course on the starboard side of the freeboard deck near the superstructure. Dented guard rails were found at the port manifold and port slop tanks. The chain was missing at the bottom course at the port pilot embarkation. ILLC Reg 25(3)(a) / ILLC Reg 25(3)(d)

07123 - The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. While conducting a test of the deck foam system the isolation valves were inoperable. 74 SOLAS (14) I/11(a)

11117 - Life saving appliances shall be fitted with retroreflective tape. PSCO observed multiple lifebuoys on deck and the rescue boat with deteriorated retroreflective tape. 74 SOLAS (14) III/34 LSA 1.2.2.7

06107 - High level alarm must be identified with the legend "High Level Alarm" in black letters at least 50 mm high on white background. PSCO observed "H" marking for the high level alarm on the superstructure. 46 CFR 39.20-7(c)(2)

06107 - Tank overfill alarm must be identified with the legend "Tank Overfill Alarm" in black letters at least 50 mm high on a white background. PSCO observed "H/H" marking for the tank overfill alarm on the superstructure. 46 CFR 39.20-7(d)(3)

The machinery shall be of design and construction adequate for the service for which they are inteded. The design shall have regard to materials used in construction, the purpose for which the equipment is intended. The vessel is intended to operate at 11 knots at normal cruising speed. Vessel master reported to company and US Coast Guard that the main engine turbo charger needs to be repaired in order to prevent high temperature alarm and reduction of speed to below 7.5 knots. 0999- Other (Safety In General)-The ship shall be divided by thermal and structural boundaries; the fire integrity of the divisions shall be maintained at openings and penetrations. 1750- (Oil/water interface detector)- A ship is subject to inspection where there are clear grounds for believing that the crew are not familiar with the essential shipboard operations relating to the prevention of pollution by oil. Vsl crew did not know how to recall information from the OCM memory and could not verify history of OWS operation IAW the provisions of MEPC 107(49). MARPOL Annex I/11 40ac
