Protective clothing shall be provided to protect the skin from the heat radiating from the fire and from burns and scalding steam. Two of the four fire-fighters outfits utilized helmets that left the fire fighters ears and neck exposed.

Testing and calibration of gas detection system should be carried out at regular intervals. Vessel test procedures did not ensure equipment is properly calibrated per manufacturers recommendations. Last calibration was carried out by the shipyard in February 2015. Equipment was calibrated to 80% LEL vice 50% LEL.
