No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on board tankers. PSCO identified numerous light fixtures throughout the engine room, including the purifier room which are fitted with incorrect light covers sacrificing the light integrity and creating a fire hazard. Master and Chief Engineer requested new covers in October 2020. The vessel has been unable to receive replacements since November 2020.

The vessel reported reduced propulsion due to a mooring line wrapped around the propeller and shaft. Provide dive survey report addressing line removal operation and any damage to vessel. Provide Classification Society report addressing verification of proper operation of machinery and shaft alignment.
