The master, owner, operator, agent, or person in charge of a vessel subject to this subpart and this section must a ballast water report to the National Ballast Information Clearinghouse (NBIC) by electronic ballast water report format using methods specified at NBIC;s Web Site at http;//invasions.si.edu/nbic/submit.html. The ballast water report will include the information listed in paragraph (c) of this section and must be submitted as follows: For any vessel that is equipped with ballast water tanks and bound for ports or places in the United States: Submit the ballast water report no later than 6 hours after arrival at the port or place of destination, or prior to departure from that port or place of destination, whichever is earlier. Vessel failed to submit a ballast water report within the required time frame. 33 CFR 151.2060(b)(3) 10c

No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on board tankers. PSCO identified numerous light fixtures throughout the engine room, including the purifier room which are fitted with incorrect light covers sacrificing the light integrity and creating a fire hazard. Master and Chief Engineer requested new covers in October 2020. The vessel has been unable to receive replacements since November 2020. SOLAS 04 cons. II-1 45.10 // 40ac

The vessel reported reduced propulsion due to a mooring line wrapped around the propeller and shaft. Provide dive survey report addressing line removal operation and any damage to vessel. Provide Classification Society report addressing verification of proper operation of machinery and shaft alignment.

Electrical installations in hazardous locaiton shall be installed with special precautions as to reduce the risk of explosion in the event of an electrical fault. PSCO's observed deterioriated electrical conduit, loose stuffing tubes and neglected wiring in the vacinity of the Cargo Dischage pumps for both the port and starboard slop tanks.74SOLAS II/1/45.5.4.
