If during an Internal structural exam damage to the hull plating, structural members, or cargo tanks is discovered, the Officer in Charge, Marine Inspection, may require the vessel to be dry docked to effect permanent repairs. The following areas were identified by the attending marine inspector (MI) to require repairs at the next scheduled dry dock, but no later than 31OCT27. Void No.5 (port): Fractured weld on tank top bracket connected to webbed frame. Void No.3 (port): Fractured weld on tank top bracket connected to webbed frame. Void No.1 (port): Fractured weld on hull plate longitudinal. 46 CFR 31.10-21(c)

Boilers, main and auxiliary machinery, and piping systems shall conform to the requirements of subchapter F (Marine Engineering) of this chapter, except as otherwise provided for in this subchapter. Repair starboard cargo pump. 46 CFR 32.35-1

46 CFR 32.50-1 Where cargo pump shafts pierce gastight bulkheads, stuffing boxes with readily accessible gastight glands shall be provided. Bolts missing from glands compromising the gas tight integrity.

46 CFR 61.20 (a) At each inspection for certification and periodic inspection the marine inspector must conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment, including the fluid control systems, as they feel necessary to check safe operation. Engine for the compressor had a fuel leak at connection to injector. Do not operate compressor engine until repairs are made to the satisfaction of the marine inspector.

The cargo handling equipment shall be maintained by the tank vessel's personnel in accordance with the regulations in this subchapter, including the following, Cargo pump pressure gage shall be tested at least once a year for accuracy. Calibrate cargo pump gauges. 46CFR35.35-70(c)

Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. Excessive wastage discovered on all cargo hatch covers and ullage openings. Crop and renew sealing surfaces and prove integrity of cargo envelope prior to the carriage of cargo, to the satisfaction of the class society.

A vessel being towed shall exhibit a stern light. The barge's stern light was inoperable at the time of inspection.

The owner, operator, or person-in-charge must ensure that portable extinguishers are of an approved type and are inspected and maintained in accordance with NFPA 10. Portable fire extinguisher #7 located in the starboard pumproom was in the incorrect bracket and the pressure gauge was showing low pressure.

Aft rake, starboard side, at aft bulkhead (FR 53), properly repair damaged areas marked by MI in 2020. Void No.5 (port): Crop and renew side longitudinal no.3 (from top) between FR 45-46. Void No.5 (port):Repair pin hole located at FR 47 located at weld between transverse watertight bulkhead and cargo tank. Void No.4 (port): Repair fractured welds (FR 39) between side shell and cargo tank. Areas are marked. Void No.3 (port): Grind and repair fractured welds located at FR 26 in-way-of side longitudinal #3 (from bottom). Void No.3 (port): Grind and repair fractured welds located at FR 25 in-way-of side longitudinal #3 (from bottom). Void No.2 (stbd): Grind and repair fractured welds located at FR 17 with cargo tank in-way-of side longitudinal #3 (from bottom). Void No.2 (port): Grind and repair fractured welds located at FR 15 and cargo tank in-way-of side longitudinal #3 (from bottom). Void No.2 (port): At FR 16, remove bottom plating rust, clean, and gauge to measure thickness. If thickness measurements are below allowable tolerances, crop and renew as necessary. Bow Rake (stbd): at FR 4, crop and renew damaged areas extending from side shell, frame, side long #1 and #2, and gunwale. Several fractured welds in area.

No cargo tank hatches, ullage holes, or butterworth plates shall be opened without flame screens, except under the supervision of the senior crew members. All cargo tank closed gauging devices consisting of a vapor sealed sight glass and level tree are broken.

Cargo tanks in which Grade A, B, or C liquids are to be transported shall be fitted with fitted with individual pressure-vacuum relief valves. The pressure-relief valve located in tank 2P was actively leaking vapor and would not sit/seal.

Flammable liquids must be stowed in containers approved by the DOT or A.S.M.E. For the contents carried, or in a portable safety container approved by a recognized testing laboratory for the contents carried. The tank barge is equipped with a portable paint locker that is loosely secured to the deck, is not gas tight, its doors do not latch and its frame is damaged.

Each cargo area enclosed by a coaming as to prevent oil from spilling into the water must be fitted with a mechanical means of closing each drain. The 5P cargo tank/slop tank is fitted with drain valves for draining oil spilled on deck. The valve was leaking cargo vapors and was not gas-tight.

Nonmetallic flexible hose must be in accordance with SE J1942. Both generating set prime movers’ are quipped with nonmetallic flexible hoses that do not contain any markings or labels as to indicate that they meet the specified standard. Provide attestation that hoses meet SAE J1942 or replace both hoses.

The #5 STBD cargo hatch was releasing air. Provide evidence of repair & pressure test the tank.

All exposed & dangerous places such as gears & machinery shall be protected with covers, guards, or rails. Guards were missing around rotating machinery. Install guards.

Piping whose temperature exceeds 150*F shall be suitably insulated. Replace lagging on exhaust for both generators.

All electrical installations in hazardous locations must comply with the requirements of IEEE. Identified dead end wire above the hatch outside of the STBD side pump room & on the STBD generator.

Cable conduits located in hazardous locations must be in suitable condition. Cable conduits found on cargo deck area were wasted & eroded.

Multiple lights in the machinery spaces were inoperable. Repair or replace.

Electrical switch on the forward mast was not intrinsically safe as required in hazardous locations. Replace.

Each battery tray must be secured to prevent movement. The battery box on the PORT side generator was not secured.

Both FWD cargo pumps showed signs of leaking at the packing gland. Provide evidence of repair.

A barge must undergo an internal examination twice in five years. A barge must undergo an annual examination every year. A barge must undergo a MARPOL ANNEX I examination every year. Inspections deferred for 90 days due to lockdown measures implemented in Puerto Rico.

Each vessel certificated under Subchapter D of Title 46, Unites States Code, must undergo an annual inspection every year after obtaining a Certificate of Inspection. Vessel's last day to complete its annual inspection was 05 Sep 2020. However, due to travel restrictions imposed to St. Maarten and St. Eustatius as a result of the COVID-19 pandemic Sector San Juan marine inspectors have not been able to attend to complete the inspection. Inspection due date has been deferred until 05 Dec 2020.

Repair all broken hatch cover hinges. (Several throughout barge)

Prove proper operation of both cargo pipe relief valves. This requirement shall be cleared within thirty (30) days.

Provide approved plans for bow thruster fuel oil transfer pump.

Renew stbd cargo pump spark arrester.

Provide suitable connector for AIS device hooked to stbd generator battery.

Mark fire extinguishers with number and location.

Prove testing of cargo pump pressure gages.

Prove semi-portable fire extinguisher is of an approved type.

Prove nonmetalic flexible hose used for the Ingersoll Rand compressor meets SAE J1942.

Prove electrical connections under the outboard generator are not wire nuts.

Prove proper operation of generator overspeed trips.

Provide MARPOL placard.

Provide PIC designation list.

Provide proof of EPIRB registration.

Prove proper operation of overspeed trips for both generators.

Prove proper operation of loss of L/O pressure for both generators.

Rapair hatch cover on portside fwd void.

Prove poper operation of relief valve on air reciever in generator space.

Conduct COI inspection, Vessel missed annual or periodic inspection IAW 46 CFR 31.10-17.

Per reg 33 CFR 155.1040(j), the VRP must be amended to either identify a classified oil spill removal organization for the Great Lakes Captain of the Port zones, or remove these zones from the plan.
