Boilers, main and auxiliary machinery, and piping systems shall conform to the requirements of subchapter F (Marine Engineering) of this chapter, except as otherwise provided for in this subchapter. Air compressor engine #2 cylinder's fuel injector has a fuel leak. Replace fuel injector. 46 CFR 32.35-1

All installations shall be of marine type engines suitable for the intended service. The starboard fuel oil pump prime mover had an exhaust leak past the turbo.

Each cargo tank on a tank barge must have a high level device to indicate visually the level of the liquid in the cargo tank when the liquid is within the range of 1 meter. The 1 meter level indicator located on tank #1stbd was not working as intended since it was stuck in the down position.

If during an Internal structural exam damage to the hull plating, structural members, or cargo tanks is discovered, the Officer in Charge, Marine Inspection, may require the vessel to be dry docked to effect permanent repairs. The following areas were identified by the attending marine inspector (MI) to require repairs at the next scheduled dry dock, but no later than 31 JAN 23: Void No.1 (port): Repair vent pipe bottom bracket at FR 12. Void No.3 (port): Repair vent pipe bottom bracket at FR 28. Void No.5 (stbd): Clean rusted areas around/adjacent to ladder. Space between FR 44-45. MI to conduct inspection of area after cleaning. Void No.3 (stbd): Replace vent pipe within space located between FR 28-29. Void No.1 (stbd): Repair pigeon holes located between FR 8-9. There are signs of wastage and small rust trails indicative of water seepage. Void No.2 (stbd): Replace damaged (marked) transverse swash bulkhead (FR 15) and adjacent side shell plating. There are broken welds and insets related to a collision.

Containment systems on board tank barges with a capacity of 250 barrels or more carrying oil as cargo in bulk must have mechanical means of closing each drain. It was noted that there is a overall practice of not installing the mechanical plugs on containment systems and on scuppers.

During each inspection all wing voids, rakes, cofferdams on tank barges shall be opened and checked from on-deck for the presence of water or cargo. The void spaces’ hatch cover knife edges and hinges are showing signs of wastage and need to be replaced/repaired at the next scheduled dry dock.

A tank vessel with a cargo capacity of 250 barrels or more that carries oil in bulk as cargo must have under each loading manifold a fixed containment so that in case of a spill it can hold its contents. It was noted that the tank barge’s starboard side manifold containment is showing concerning signs of wastage and corrosion. Repair at the next scheduled dry dock.

The annual inspection shall include an inspection of the structure is adequate and satisfactory for the route and service intended. It was noted that the starboard side aft stairs leading into the raised walkway aft of the machinery space are wasted and broken.

Exhaust pipe installations must conform to the requirements of ABYC P-1. The port side fuel oil pump prime mover’s exhaust piping is showing signs of leaks as evidenced by the soot accumulated on the lagging.

The STBD side cargo pump piping showed signs of leaking forward of the pump house. Provide evidence of repair.

The void spaces shall be provided with gooseneck vents. The gooseneck vent pipe under the #2 STBD hatch for void space #1 was not secured at the bottom.

The void spaces shall be provided with gooseneck vents. The gooseneck vent pipe to the #5 PORT side void space was severely deteriorated at the base on the deck.

Cable conduits located in hazardous locations must be in suitable condition. Cable conduits found on cargo deck area were wasted & eroded.

All exposed & dangerous places such as gears & machinery shall be protected with covers, guards, or rails. Guards were missing around rotating machinery. Install guards.

All lifesaving appliances and arrangements on tank vessels must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Gasket on self igniting light was wasted and attached with incorrect line type. Replace or repair to manufacturer's requirements.

The majority of void space hatch covers & dogging devices were broken or deteriorated. Repair or replace. Provide evidence of weathertight integrity.

The FWD rake hatch cover is missing a piece of the seal. Repair or replace.

Piping systems not requiring plan review may be accepted by the marine inspector if there are guards, shields, insulation, and similar devices where needed for protection of personnel. The marine inspector noted there was a missing section of insulation on the exhaust piping elbow past the turbocharger on both prime movers for the barge’s generators.

Install missing ladder rung on vessel’s port side permanent ladder installation.

Install missing handrail at top of permanent ladder installation.

Void Space #5 (Port side): Crop and renew side shell plating at frame 33 and side long #3.

Void Space #5 (Port side): By aft ladder, clean whole bay bottom plating, grind rusted areas, look for damage (if any), and recoat.

Void Space #3 (Port side): Crop and renew swash bulkhead at side long # 3 and 4 (buckled). Frame 25.

Void Space #2 (Port side): Crop and renew side long #3 between frames 14 and 15.

Space#1 (Port Side): At frame #7, grind and re-weld small fractures on web frame and connection with cargo tank exterior.

Void Space #1 (STBS Side): Crop and renew ladder connection (forward hatch ladder), adjacent side long (#7 side long) and tripping bracket connecting with forward transverse watertight bulkhead.

Repair all damaged void spaces’ hatch hinges.

Prove proper operation of the stern light.

Repair broken hinges on the starboard #2 aft and #3 forward voids (46 CFR 42.15-40(a)).

Prove proper operation of stbd aft MGO relief valve.

Prove proper operation of port aft MGO relief valve

Repair all broken wing voud hatch hinges.

Prove proper operations of all louver closure fastners

Replace all broken glass emergency glass emergency supply vent shut off.

Per reg 33 CFR 155.1040(j), the VRP must be amended to either identify a classified oil spill removal organization for the Great Lakes Captain of the Port zones, or remove these zones from the plan.
