The cargo handling equipment shall be maintained by the tank vessel's personnel in accordance with the regulations in this subchapter, including the following, Cargo pump pressure gage shall be tested at least once a year for accuracy. Calibrate 04 cargo pump gauges.

Cargo hose stored on board the vessel which is used in transferring cargoes listed in Table 151.05 shall be inspected every 2 years. This inspection shall consist of a visual examination and a hydrostatic test of 1 1/2 times the maximum pressure to which the hose will be subjected in service. The date of the most recent inspection and the test pressure shall be stenciled or otherwise marked on the hose. Cargo hose and cargo piping is overdue for hydrostatic test.

When minor alterations or minor repairs of tank vessels become necessary such work shall be under the direction of the Officer in Charge, Marine Inspection, and shall be in accordance with the regulations in effect at the time the vessel was contracted for or built, or in accordance with the regulations in effect for new construction insofar as possible. Marine Inspector identified a broken ladders and wasted hatch openings to the port and stbd fwd lube oil tanks. The lube oil cargo pumps remote shutdowns were not tested. Additionally, the cargo pipe leading from the forward deck tanks is wasted and holed near the port manifold. On deck Lube oil cargo tanks shall not be used until repairs are made.

All exposed & dangerous places such as gears & machinery shall be protected with covers, guards, or rails. Guards were missing around rotating machinery. Install guards. Identified dead end wires on STBD side generator.

Piping whose temperature exceeds 150*F shall be suitably insulated. Replace lagging on exhaust for both generators.

All electrical installations in hazardous locations must comply with the requirements of IEEE. Identified exposed electrical wiring on the FWD mast.

Electrical switch on the forward mast was not intrinsically safe as required in hazardous locations. Replace.

Each battery tray must be secured to prevent movement. The battery box on the PORT side generator was not secured.

Each internal combustion engine shall be provided with ventilation to allow for proper operation & maintenance. Identified an exhaust leak during test on the STBD side cargo pump engine. Repair.

The sidelights shall be fitted with matte black inboard screens. The port & starboard sidelights had screens painted red & green.

All lifesaving appliances and arrangements on tank vessels must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Gasket on self igniting light was wasted and attached with incorrect line type. Replace or repair to manufacturer's requirements.

Void Space No.1 (Port): Crop and renew port side No. 5 side long between web frames 5 and 6.

General: Repair broken hatch covers hinges throughout barge.

Vessel shall remove or comply with annual decal servicing requirements within 30 days.

Prove proper operation of cargo pump relief valves.

Per reg 33 CFR 155.1040(j), the VRP must be amended to either identify a classified oil spill removal organization for the Great Lakes Captain of the Port zones, or remove these zones from the plan.
