The fire resistance of doors shall be equivalent to that of the division in which they are fitted. Galley door that leads to scullery cannot self close or latch due to improper installation. Door was also found fitted with hold back hook. 74SOLAS(20)II-2/9.4.2.1

No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on board tankers or in compartments assigned principally to accumulator batteries, in paint locker lockers, acetylene stores or similar spaces, unless the Administration is satisfied that such equipment is: appropriately certified for safe usage in the dusts, vapours or gases likely to be encountered. Vessel’s lighting fixture located outside the acetylene and oxygen lockers was installed with inadequate fittings (i.e. t-coupling, elbow, and nipples) that do not meet the explosion proof requirements. 74SOLAS(04)II-1/45.10.4

The Administration shall take appropriate steps to ensure uniformity in the implementation and application of the provisions of this part in respect of electrical installations. Cable gland and wiring to the deck water seal float switch was installed with improper fittings and has wiring exposed(i.e. t-glads, coupling,) that do not meet the explosion proof requirements identified by the International Electrotechnical Commission (IEC) Publication 92. 74SOLAS(04)II-1/40.2

Electrical installations shall be such that: electrical services essential for safety will be ensured under various emergency conditions. Multiple emergency lights were observed to be not operational near embarkation areas for the lifeboat, rescue boat, and liferafts. 74SOLAS(04)II-1/40.1.2

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and rady for immediate use. The self contained air support system for the free fall lifeboat had a broken gauge rendering it unreadable and was leaking air into the boat at an uncontrollable rate. 74SOLAS(20)III/20.2

Electrical installations shall be such that electrical services essential for safety will be ensured under various emergency conditions. While testing the standby function for the no. 3 CFW pump running in auto change mode, the pump was not able to start up automatically. 74SOLAS(04)II-1/40.1.2

A main source of electrical power of sufficient capacity to supply all those services mentioned in regulation 40.1.1 shall be provided. The main source of power shall consist of at east two generating sets. Vessel reported failure of main circuit breakers for No.2 and No.3 main diesel generators. Vessel to conduct repairs to the satisfaction of RO. 74 SOLAS (41) II-1/41.1.1

Fire Protection systems and fire-fighting systems and appliances shall be maintained ready for use. Vessels water mist system indicating system fault relating to the Pump Starter Motor. 74 SOLAS(08)II-2/14.1.1

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Auxiliary boiler inoperable at time of inspection, unable to complete testing of equipment. 74 SOLAS(08)II-1/26.1

Illustrations and instructions in appropriate languages shall be posted in passenger cabins and be conspicuously displayed at muster stations and other passenger spaces to inform passengers of: the method of donning lifejackets. Marine Inspector discovered missing lifejacket donning instructions at lifeboat muster station. 74 SOLAS(20)III/8.4

Waste receptacles shall be constructed of noncombustible materials with no openings in the sides of bottom. Several wastes receptacles throughout the ship were made of combustible materials(plastic). 74 SOLAS(08)II-2/4.4.2

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Ring buoy light located on the starboard cargo deck handrail.

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors that are weathertight/watertight when closed. Marine inspector noticed light coming through the weathertight/watertight doors within the superstructure when fully dogged down. ILLC(98)Annex I Chapter II/12

The fire integrity of the divisions shall be maintained at openings and penetrations. Cable's penetrating fire divisions were incorrectly installed inside cable trunk on “D” deck. 74 SOLAS(08)II-2/9.1.3

Protection of cargo tanks shall be achieved by a fixed inert gas system in accordance with the Fire Safety Systems Code(FSS) requiring the pressure of inert gas be continuously indicated. Vessels Kongsberg inert gas control screen is showing incorrect pressure readings for NO.2 C.O.T(P).

The inert gas system referred to in paragraph 5.5.3.1 shall be designed, constructed and tested in accordance with the Fire Safety System Code. In addition to the requirements of paragraph 2.2.4.5, audible and visual alarms shall be provided to indicate: low water pressure or low water flow rate to the cooling and scrubbing arrangement. The low pressure cooling water inlet alarm was inoperable. 705 A- Prior to cargo operations. 74 SOLAS (04) 11-2/4.5.5.3.2 FSSC/15.2.3.2.2.3

A general emergency alarm system complying with the requirements of paragraph 7.2.1 of the Code shall be provided and shall be used for summoning passengers and crew to muster stations and to initiate the actions included in the muster list. The system shall be supplemented by either a public address system complying with the requirements of 7.2.2 of the Code or other suitable means of communication. The public address system shall be a loudspeaker installation enabling the broadcast of messages into all spaces where crew members or passengers, or both, are normally present, and to muster stations. The public address system on the Fore Mast was inoperable.

The arrangements shall ensure that the tightness can be maintained in any sea conditions. The means for securing and maintaining weathertightness shall be to the satisfaction of the Administration. MI observed the Port and Starboard weathertight doors of the A-Deck will not dog down. ICLL 88/16.1

All hatchways in position 1 and 2 shall be fitted with hatch covers of steel or other equivalent material. Such covers shall be weathertight and fitted with gasket and clamping devices. The arrangements shall ensure that the tightness can be maintained in any sea conditions. The means for securing and maintaining weathertightness shall be to the satisfaction of the Administration. Marine Inspector identified vent closure approximately 12” x 12” located on the bulkhead above and left of the Class I weathertight door for the Emergency Diesel Generator compartment with severely corroded/wasted knife edge for sealing.

All hatchways in position 1 and 2 shall be fitted with hatch covers of steel or other equivalent material. Such covers shall be weathertight and fitted with gasket and clamping devices. The arrangements shall ensure that the tightness can be maintained in any sea conditions. The means for securing and maintaining weathertightness shall be to the satisfaction of the Administration. Marine Inspector identified knife edge on the Class I weathertight door for the Emergency Diesel Generator compartment that is corroded/wasted located on the bottom right of knife edge approximately 4” above the turn. ICLL 88/16.1

All hatchways in position 1 and 2 shall be fitted with hatch covers of steel or other equivalent material. Such covers shall be weathertight and fitted with gasket and clamping devices. The arrangements shall ensure that the tightness can be maintained in any sea conditions. The means for securing and maintaining weathertightness shall be to the satisfaction of the Administration. Marine Inspector identified Class I watertight door for the Emergency Diesel Generator compartment with missing/corroded metal track for gasketing. ICLL 88/16.1

Every able seafarer deck serving on a seagoing ship of 500 GT or more shall be required to demonstrate the competence to perform the functions at the support level, as specified in column 1 of table A-II/5. The minimum knowledge, understanding and proficiency required of an able seafarer deck serving on a seagoing ship 500 GT or more is listed in column 2 of table A-II/5. The Ordinary Seaman handled lines during mooring operations without the proper STCW Endorsement (II/5). The vessel is appropriately manned with adequate personnel in accordance with the COI, however member should not have been utilized in the line handling position. STCW Part A-II/5.1 and 5.2

All persons who are assigned duty as officer in charge of a watch or as a rating forming part of a watch and whose duties involve designated safety, prevention of pollution and security duties shall be provided with rest period of not less than a minimum of 10 hours of rest in any 24 hour period. Hours of rest shall be at least 6 hours in length. As per the vessel's system (Watchkeeping) the C/E on NOV 23, 2022 did not receive 10 hours of rest in the previous 24 hour period and did not receive the minimum 6 hour rest period. STCW Part A-VIII/1.2 and 1.3

The machinery, boilers, and other pressure vessels associated piping systems, and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. Marine Inspector identified the #2 Framo engine for the cargo pump fuel oil supply hose to the racor filter assembly is dry rotted. At the fitting at the racor assembly the hose is cracked. Engineers replaced hose and demonstrated proper operation of the engine with no leaks and no other concerns. 74 SOLAS (04) II-1/26.1

74 SOLAS (14) IX/3.1 ISM Code 10.1 A 30 The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. Master was unfamiliar with the contents of the vessel's SMS and unable to provide inspection reports, completed checklists for routine maintenance, or objective evidence that inspections and maintenance is being conducted to the watertight hatches and closures. Further, the Master was unable to locate SMS related material requiring assistance from the company in locating the SMS material. Due to the objective evidence in the below deficiencies, the vessel is not in substantial compliance with relevant conventions, calling into question the adequacy and implementation of the vessel’s SMS under the ISM code. An external audit conducted by the RO is required to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the completed audit report to USCG prior to departure from port.

ICLL (88) 16.1 A 30 All hatchways in position 1 and 2 shall be fitted with hatch covers of steel or other equivalent material. Such hatch covers shall be weather tight and fitted with gaskets and clamping devices. The arrangements shall ensure that the tightness can be maintained in any sea conditions. MI observed the watertight hatch for the Bosun space on the bow was not watertight. MI could feel air egressing from the space when dogged down. Crew conducted a hose test at 30 PSI which resulted in water ingress. MI observed the goose neck located at the Bow Centerline has irregularity on the knife edge preventing closure.

ICLL (88) 19.4 A 30 Ventilator openings shall be provided with weather tight closing appliances of steel or other equivalent material. MI observed the natural ventilation mushroom head for the Bosun Store on the Starboard Bow is wasted and holed. MI observed the gasket for the power ventilation mushroom head for the Pant Stores wasted and corroded.

74 SOLAS (04) II-1/ 26.1 A 50 The machinery, boilers, and other pressure vessels associated piping systems, and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The #1 Framo cargo pump fuel oil supply hose is dry rotted/ cracked.

74 SOLAS (04) II-1/45.1.1 A 50 Exposed metal parts of electrical machines or equipment which are not intended to be live but which are liable under fault conditions to become live shall be earthed. MI observed broken grounding wires on the Starboard side rescue boat muster station lighting. In addition, grounding wires to the controls of the Rescue Boat davit are corroded and broken. The power ventilation for paint and lamp stores has a disconnected ground wire. The tank heater locker power ventilation has a disconnected ground wire.

33 CFR 155.730 A 50 The vessel operator of each vessel required by § 155.720 to have transfer procedures shall maintain them current and shall require vessel personnel to use the transfer procedures for each transfer operation. Section 2, page 6 of vessel's transfer procedure requires " Top dead centerr of each flange bolt circle is a preventer pin designed to further identify these flanges as vapor recovery connections and not for the transfer of liquid cargo or ballast." Vapor recovery flange did not have a stud.

74 SOLAS (14) III/ 20.2 C 10 Before the ship leaves port and at all times during the voyage, all life-saving appliance shall be in working order and ready for immediate use. MI observed the Port Bow embarkation ladder tie down strap was seized rendering it inoperable. In addition, securing shackles of the ladder were seized and unable to operate.

74 SOLAS (14) II-2/ 10.4 10 C Ships constructed before 1 JUL 2014 shall comply with the requirements of this paragraph not later than the first survey after 1 JUL 2018: a minimum of two-way portable communications radiotelegraph apparatus for each fire party for the fire-fighter's communication shall be carried on board.

A food ration totaling not less than 10,000 kJ for each person the lifeboat is permitted to accomodate. The rations shall be packed in a permanently sealed or vaccuum packed in a flexible packaging material. 10 out of 32 food rations found on board the lifeboat have lost their vaccuum seal. Replace food rations with new rations. SOLAS 74 (2020 Con. Ed.) III/13.1.4, LSA Code (2010) Ch. IV/4.4.8.12

Any main and auxiliary steering gear control system operable from the navigation bridge shall comply with the following: The system shall be capable of being brought into operation from a position on the navigation bridge. Vessel experienced a non follow up of rudder on port steering pump. Provide transit plan and class report attesting to proper operation of steering system.
