Whenever a defect is discovered which effects equipment covered by the Code, the Master shall report to the RO responsible for issuing the Certificate and PSC Authorities; Vessel's 3P Framo pump stopped working. Vessel is authorized to utilize portable cargo pump for discharge IAW P&A Manual and Risk Assessment. Vessel is not authorized to replace Framo pump until Flag and Class have approved procedures. Prewash will not be completed prior to departure IAW MARPOL Annex II 13.4.3

Resolution: Vessel conducted maintenance on pump and was able to clear the deficiency without replacing or using spare parts.

Resolved 4 February 2021
Resolution: The ship received replacement spare parts and installed them. Upon our arrival we tested the opertion of the rescue boat fully in ahead and astern directions as well as full throttle and find repairs acceptable. The outboard motor is working as designed.

A portable foam applicator unit shall contain at least 20 liters of foam concentrate and at least one spare tank of foam concentrate of the same capacity. Portable foam applicator unit near generator #2 was not provided with a spare tank of foam concentrate. 74SOLAS(14) II-2/10.5.2.2.1 Ref FSS Code 4.3.2.1. 16ac.

A chemical tanker shall comply with the requirements of the IBC Code. Cargo tanks shall be fitted with visual and audible high level and tank over-flow alarms. Audible alarm for high level and tank overflow was inoperable. 74SOLAS(14) VII/10.1 Ref IBC Code 15.19.

Resolved 9 March 2019
Resolution: Siren's supply of ship service air had been cut off for maintenance. Ship service air supply was re-engaged and the alarm was observed to be in proper working condition.

0799- The inert gas system shall be capable of inerting, purging, and gas-freeing empty tanks and maintaining the atmosphere in cargo tanks with the required oxygen content. PSCO observed the calibration gas for the inert gas generator was expired and crew was unable to prove accurate nitrogen purity levels. Correct prior to carrying cargoes requiring inert gas as required. 74SOLAS(04) II-2/Reg.4.5.5.3.1 Action Taken 40ac
