The machinery, boilers and other pressure vessels, associated piping systems and fitting shall be of a design and construction adequate for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board.
While transiting outbound along the Houston Ship Channel the vessel experienced a loss of propulsion due to a leak of cooling jacket water.
Condition: Improper/Lack of Maintenance
Action required: 60 - Rectify deficiencies prior to movement
Resolved 28 February 2020
Resolution: Received, reviewed and accepted class report from ABS stating the deficiency was rectified. Crew replaced No.4 cylinder cover with original equipment and was tested satisfactory.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company.
The vessels "Machinery Running Hours" documents lists "CYL Cover since O'Haul" as requiring maintenance every 18,000 running hours. The same Document lists Cylinders #4 and #5 as having 23,037 hours each as of 31JAN2020.
Due to the above Deficiency an internal safety audit focusing on machinery maintenance is required.

Resolution: Company submittd documentation attesting to the SMS audit. The internal Investigation revealed no breach of company policy, maintenance instructions, makers instructions or wrongly executed maintenance or Main Engine Operation.
