Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 mm (.5 inch) diameter and is at least 25.4 mm (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. Permanent studs at the fwd vapor line on the port side manifold and the aft vapor line on the starboard manifold were missing.

Mixed garbage was found in various parts of the vessel. Also the garbage plan did not address the disposal of medicines, batteries or containment of ash. All of these items were being kept on board and segregated but they have not been identified in their plan.

Controlled drugs were found in the refrigerator in the hospital space. The door to the refrigerator was not locked. According to the World Health Organization the recommendation is that the drugs (Atropine) should be kept refrigerated. The drugs were taken out and placed in the Captain’s safe until the ship's crew can place a hasp and lock on the door, ILO 147.

The Ballast Water Management Plan is not ship specific. The plan is a mixture of items form the IMO Res. 868 (20), and incorporating items relating to the ship. But it doesn’t come close to being ship specific. Crew training is not being done, safety measures of when not to exchange ballast have not been addressed and the specific number of tanks or pumps are not listed.
