Except as provided in paragraph (a)(8) of this section, each towing vessel must have a fire-detection system installed to detect engine room fires. Fire detection panel in wheelhouse displaying yellow ‘trouble’ light. Determine cause of trouble light and prove functionality of fire detection system. 46 CFR 142.330

The owner, master or officer in charge of a towing vessel must ensure the following: The size, material, and condition of towlines, lines, wires, push gear, cables and other rigging. Port aft winch wire found with several broken strands. 46 CFR 140.801

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Breaker panel located in the wheelhouse missing dead front cover. 46 CFR 143.400(a)

Each towing vessel operating on oceans, coastwise, or Great Lakes routes must have a means to take blood pressure readings, splint broken bones, and apply large bandages for serious wounds. First aid kit was incomplete. 46 CFR 140.435

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Wheelhouse port side watertight door had wasted gasket. 46 CFR140.610(b)

At least one lifebuoy must have a lifeline, secured around the body of the lifebuoy. If more than one lifebuoy is carried, at least one must not have a lifeline attached. Lifeline was attached to both lifebuoys on board. 46 CFR 141.360(c)(1)

Stowage positions for lifejackets stowed in a berthing space or stateroom and all lifejacket containers must be marked in block capital letters and numbers with the minimum quantity, identity, and, if sizes other than adult or universal sizes are used on the vessel, the size of the lifejackets stowed inside the container. Number of lifejackets was not posted at stowage location. 46 CFR 141.340(h)

The name of the towing vessel must be marked or painted in clearly legible letters on each EPIRB. EPIRB onboard missing name. 46 CFR 141.380

The following minimum information is required in the record: The dates when examinations were performed, theidentification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. Towing gear log did not have standard entries for condition of the gear. 46 CFR 140.820(b)

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. Navigation assessments containing information in 140.635 were not available for inspection. 46 CFR 140.635

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Two portholes onboard found permanently open due to A/C ducting rigged to the window. Port cover for head forward of engine room on starboard side found missing one dog for closure. 46 CFR 144.610

On a merchant vessel of the United States the construction of which began after March 4th, 1915..(5) each crew berthing area shall be equipped with information regarding-(A) vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage; and (B) procedures and resources to report crimes, including sexual assault and sexual harassment, including information-(i) on the telephone number, website address, and email address for reporting allegations of sexual assault and sexual harassment to the Coast Guard;(ii) on vessel owner or company procedures to report violations of company policy and access resources; (iii) on resources provided by outside organizations such as sexual assault hotlines and counseling;(iv) on the retention period for surveillance video recording after an incident of sexual harassment or sexual assault is reported; and (v) additional items specified in regulations issued by, and at the discretion of, the Secretary of the department in which the Coast Guard is operating. (d) In each washing space in a visible location there shall be information regarding procedures and resources to report crimes upon the vessel, including sexual assault and sexual harassment, and vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage. Vessel missing required signage in crew berthing areas. 46 U.S.C. 11011(a)

An existing vessel may be deemed by the OCMI to be in compliance with this subpart provided that either: the vessel is built, equipped, and maintained to conform to the rules of a recognized classification society appropriate for the intended service and routes. Three vertical stiffeners on aft bulkhead of forepeak found holed or wasted and require renewal. 46 CFR 144.200

Survival craft must be stowed in accordance with the requirements in 199.130. Each liferaft must be arranged for float-free launching. The arrangement must ensure the liferaft when released and inflated are not dragged under by the sinking vessel. IBA painter line was made off to the storage bracket and not connected to the weak link. 46 CFR 141.310 46 CFR 199.130

The size, material, and condition of towlines, lines, wires, push gear, cables, and other rigging used for making up a tow or securing the towing vessel to a tow must be appropriate for: The static loads and dynamic loads expected during the intended service. Vessel’s towing wire on port side was frayed. Replace to the satisfaction of the MI. 46 CFR 140.801 (b)(2)

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. The port, aft watertight door had a crack. Repair to the satisfaction of the MI. 46 CFR 140.610 (b)

Each watertight door and watertight hatch must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR—KEEP CLOSED” or “WATERTIGHT HATCH—KEEP CLOSED”. Watertight doors did not have markings on both sides. Add markings to the satisfaction of the MI. 46 CFR 140.160

The examination must address the items in § 137.330 as applicable, and must include a visual examination of the external structure of the vessel to confirm that the condition is properly maintained. Observed overhead wastage on port side of engine room located on either side of 1st overhead stringer forward of fuel tank, approximately 12” inboard of the side shell. Opposite side of wastage is located on the weather deck and appears to be within a closed portion of bulwark. Repair prior to next credit dry dock. 46 CFR 137.325

A hydrostatic release unit used in a float-free arrangement must be approved under approval series 160.162. Marine inspector found expired hydrostatic release unit in use on liferaft. Vessel to replace with new unit to the satisfaction of the marine inspector prior to movement. 46 CFR 141.310 46 CFR 199.130(c)(7)

Each towing vessel operating on oceans, coastwise, or limited coastwise must carry a Category 1, 406 MHz satellite Emergency Position Indicating Radio Beacon (EPIRB) that meets the requirements of 47 CFR part 80. When the towing vessel is underway, the EPIRB must be stowed in its float-free bracket with the controls set for automatic activation and be mounted in a manner so that it will float free if the towing vessel sinks. The owner or managing operator must maintain valid proof of registration. Marine inspector found EPIRB had expired hydrostatic release unit, expired battery, and expired registration. Vessel to replace all three to the satisfaction of the marine inspector prior to departure. 46 CFR 141.380

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. Marine inspector found expired line throwing appliance. Vessel to replace with new unit to the satisfaction of the marine inspector. 46 CFR 141.385

All portable fire extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months. Vessel to replace battery in fire-detection system as indicated on servicing report to the satisfaction of the marine inspector. 46 CFR 142.240(a)

Electrical components that pose an electrical hazard must be in an enclosure. Marine inspector found exposed electrical connections and loose wiring in multiple locations. Vessel to remove or enclose all such wiring to the satisfaction of the marine inspector. 46 CFR 143.400(g)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Marine inspector found the use of 'daisy chains' in multiple locations. Vessel to remove all 'daisy chains'. 46 CFR 143.400(a)

Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. Marine inspector found battery chargers for port and starboard main engines and generators of the plug in type. Vessel to hard wire all battery chargers to the satisfaction of the marine inspector. 46 CFR 143.400(e)

The inspection will ensure that the vessel is in satisfactory condition and fit for the service for which it is intended. The inspection will include inspections of the structure, pressure vessels and their appurtenances, piping, main and auxiliary machinery, electrical installations, lifesaving appliances, fire detecting and extinguishing equipment, pilot boarding equipment, and other equipment. Marine inspector found sewage pipe wrapped in duct tape and showing signs of leakage. Vessel to repair pipe to the satisfaction of the marine inspector. 46 CFR 136.212(b)

Portable and semi-portable fire extinguishers must be inspected, maintained, and tested in accordance with the inspection, maintenance procedures, and hydrostatic pressure tests required by Chapters 7 and 8 of NFPA 10, Standard for Portable Fire Extinguishers (incorporated by reference, see § 136.112 of this subchapter). Marine inspector found portable fire extinguishers mounted in improperly sized brackets. Vessel to correct bracket size to the satisfaction of the marine inspector. 46 CFR 142.240(a)(1)

Except as provided in paragraph (a)(4) of this section, each towing vessel operating north of lat. 32° N. or south of lat. 32° S. must carry the number of immersion suits as prescribed in this paragraph (a): (1) Each towing vessel operating in those regions must carry at least one appropriate-size immersion suit, approved under approval series 160.171, for each person onboard. Marine inspector found immersion suits on board greater than 10 years old. Vessel to service immersion suits in accordance with manufacturers instructions to the satisfaction of the marine inspector. 46 CFR 141.350(a)(1)

Individual circuit breakers on switchboards and distribution panels must be labeled with a description of the loads they serve. Labels on the distribution panel and the DC Fuse box on the bridge are missing. 46 CFR 143.400(d)

Each vessel must be designed and constructed to minimize fire hazards insofar as reasonable and practicable. Fuel tank sight glasses need to be changed from plastic to glass to minimize fire hazards. 46 CFR 144.405

An existing vessel may be deemed by the OCMI to be in compliance, provide the vessel is built, equipped, and maintained for the intended service and routes. A crack in an overhead frame at centerline in the fidley was found and needs repaired prior to credit drydock. 46 CFR 144.200

Each mattress must comply with either The Consumer Safety Commission Standard for Mattress Flammability or IMO Resolution A.688(17). Vessel mattresses did not have indication they meet flammability requirements.

The results of inspections required by 33CFR164.76 must be documented in the TVR or official logbook. Vessel only had one month's worth of towing gear inspection records. 46 CFR 140.820(a)

Each towing vessel must carry at least one appropriately-sized lifejacket for each person on board. Vessel is limited to only carrying 4 crew until the 2 additional lifejackets are on board.
