Pressure sensors may be fitted in each tank protected by the arrangement required in Paragraph 6.1.2 with a monitoring system in the ships CCR or the position where cargo operations are normally carried out. Such monitoring equipment shall also be provide an alarm facility which is activated by detection of over-pressure or under-pressure conditions in the tank. PSCO observed audible monitoring system alarm for tank over-pressure manually secured in CCR due to reoccurring ground fault alarm in system. 74 SOLAS (04) II-2/11.6.3.2

Each tank ship with a vapor collection system must be fitted with a pressure-sensing device…that has a high-pressure alarm that activates when the pressure-sensing measures a high pressure of not more than 90 percent of the lowest pressure relief valve setting in the cargo tank venting system. PSCO observed dry-dock report stating all P/V valves were set to 1400mmWG, despite being labeled 1700mmWG locally on board. Due to this inconsistency, high pressure alarm on board is set to 1530mmWG, now above the P/V valve setting. 46 CFR 39.2013(b)(2)

The machinery, boilers, other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO observed oil leaks on generators 1, 2, and 3. Additionally leaks were found on the main engine at fuel pumps on cylinders 2, 3, 4 and 5. 74 SOLAS (04) II-1/26.1

Resolution: Number 2 pump suction line partly cropped and piece of same material grade steel inserted. Repairs by certified welder w/ approved welding consumables at sea, non-destructive examination by liquid penetrate method and hydrostatic test found satisfactory.
