13104 - A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. The visual indicator was not operating correctly and needs to be fixed. 46 CFR 182.530(c)

11118 - The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. The number and sizes of life jackets stowed beneath the seats were not marked. 46 CFR 185.604(f)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed Vessels davit failed, causing vessel to drop bow first into water resulting in unknown damage. Additional inspection required prior to departure to the satisfaction of attending Marine Inspector.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Vessel sustained damage to hull. Additional inspection required prior to departure to the satisfaction of attending Marine Inspector.

Each vessel certificated to operate on a limited coastwise route within three miles of land in cold water must be provided with the survival craft required by 46 CFR 180.204(d) which states that a vessel must be provided with either life floats of an aggregate capacity that will accommodate at least 100% of the total number of persons permitted on board or have on board an FCC Type Accepted Category 1 406 MHz EPIRB, installed to automatically float free and activate, and be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted on board. Vessel arrangement does not currently allow for installation of life floats. Vessel is limited to protected waters, not more than 1 mile from land during cold water months. Cold water months are defined by the cognizant OCMI. The vessel's Certificate of Inspection will be amended to reflect this change.

Each vessel certificated to operate on a limited coastwise route within three miles of land in warm water must either be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted or have on board an FCC Type Accepted Category 1 406 MHz EPIRB, installed to automatically float free and activate, and not be required to carry survival craft. Vessel does not have life floats or an EPIRB on board. Until this deficiency is rectified, AMERICAN STAR must maintain radio communications and visual/radar contact with the LAUNCH 6 while it is operating on a limited coastwise route in warm water; where the monthly mean low water temperature is greater than 59 degrees Fahrenheit. Warm water months are defined by the cognizant OCMI.

At least one life ring buoy must be fitted with a lifeline. Ring life buoy has no life line.

An adult life jacket must be provided for each person on board a vessel. 22 life jackets found in unservicable condition. Replace 22 life jackets.

Portable tanks must meet the requirements of ABYC H-25. Vessel has 03 portable tanks: 01 5-gallon, 01 7-gallon, and 01 12-gallon. Remove excess fuel tanks to meet ABYC H-25 requirements.

All vessels must have navigation lights that are in compliance with applicable sections of the International and Inland Navigation Rules. Starboard side light and stern light found not permanently affixed.

Electrical equipment on a vessel must be installed to protect vessel, passengers and crew from electrical hazards. Port side aft deck lights had dead end wires.

Vessels over 26 feet in length must be provided with an alarm, audible and visual, to indicate a high water level. High water level alarm not properly installed.

A vessel must carry charts and publications for the intended voyage. No charts or publications on board.

Emergency instructions shall be mounted on the operating station. Emergency instructions were not posted on operating station nor were tailored to the vessel.

Radio operator license or permit of class required. Operator does not have radio operator permit. 47 CFR 80.159(e)

A vessel must comply with the applicable requirements for any radio installation. VHF radio does not have MMSI number entered.

Each vessel must be equipped with at least two operable portable battery lights. Missing portable lights.

A vessel of less than 12 meters in length shall not be obliged to carry the sound signaling appliances prescribed in paragraph (a) of this Rule but if she does not, she shall be provided with some other means of making an efficient sound signal. Vessel's horn did not work.

A vessel of more than 7.9 meters (26 feet) in length, but not more than 19.8 meters (65 feet), must carry a minimum of one life buoy of not less than 610 millimeters (24 inches) in diameter. Vessel ring buoy was not serviceable.

The certificate of inspection (COI) issued by an Officer in Charge, Marine Inspection (OCMI), to a vessel required to be inspected under 46 U.S.C. 3301 specifies the minimum complement of officers and crew necessary for the safe operation of the vessel. Vessel was not manned in accordance with their COI.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery including operation of the main propulsion machinery both ahead and astern. Vessel's main propulsion outboard motor does not turn on and operate.

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Vessel was not crewed. Therefore, drills could not be completed.

A vessel not required to have a power driven fire pump by §181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel does not have 03 fire buckets.

Each battery used in an item of lifesaving equipment, except inflatable survival craft equipment, must be replaced during the annual inspection if the expiration date of the battery has passed. Vessel's waterlight battery is expired.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. The bilge alarm does not work.

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Vessel was not crewed and could not provide DAPI information.

A visual high-water alarm indicator must be located at the operating station. The visual high-water alarm indicator is inoperable.

Provide proof of valid FCC Safety Certificate.

Sidelights mean a green light on the starboard side and a red light on the port side each showing an unbroken light over an arc of the horizon of 112.5 degrees. Vessel has a broken red light on the port side.

Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose. Vessel has zip ties on 4 different ends of the fuel system acting as second hose clamp. Remove zip ties and replace with hose clamps.
