Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. MI observed cable penetrations in STBD Laz with mixed and torn fire-stop putty. Repair to the satisfaction of the MI. 46 CFR 116.415 (a)(2)

Electrical equipment on a vessel must be installed and maintained to: Minimize accidental personnel contact with energized parts. Port engine room generator was missing a face plate. Repair to the satisfaction of the MI. 46 CFR 120.200 (c)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. The port engine room contained flammable liquid in an open container. MI observed the removal of the oil from the vessel during the inspection. 46 CFR 115.830 (a)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. The vessel experienced a loss of propulsion while transiting to San Francisco, CA. Vessel to identify extent of the issue, root cause and to conduct repairs to the satisfaction of the OCMI. 46 CFR 119.100

The emergency lighting required must automatically actuate upon failure of the main lighting system. Port and STBD Engine room emergency lights failed to actuate upon loss of normal power. 46 CFR 120.432 (b)

An enclosed or partially enclosed space within a vessel must be adequately ventilated in a manner suitable for the purpose of the space. Aft STBD vent cover for the transformer space is not attached properly. 46 CFR 116.600(a)

Each device must— (1) Be free of design defects such as rough or sharp edges that may cause bodily injuries or that would allow toxic substances to escape to the interior of the vessel. Investigate sewage piping for toxic substances leak. 33 CFR 159.95(a)(1)

A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Sewage void cable transit has unused cables blocking the penetration. 46 CFR 179.320(c)

Each rescue boat and ring life buoy must be marked with Type II retro reflective material.. Port aft life ring was missing type II retro reflective tape. 46 CFR 122.604 (i)

Starboard engine began smoking while underway leaving the dock, crew secured starboard engine and returned to the dock under the port engine. Provide report of cause analysis and conduct repairs to the satisfaction of attending Marine Inspector prior to movement. 46CFR119.200(a)

A vessel's stability letter must be posted under glass such that all pages are visible at the operating station of the vessel. Current stability letter not posted in operating station.

The master and crew of a vessel will have mounted at the operating station, emergency instructions containing actions to be taken in the event of fire, flooding, heavy weather, or man overboard. Vessel did not have emergency instruction posted in operating station.

Provide 19 more type I PFD's, vessel restricted to 211 passengers.

Provide flashlights near the entrance to both engine rooms.

Prove proper operation of starboard bilge pump.
