Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gastight and self-closing. Boundary doors between main Eng space and steering gear room/storage rooms are missing locks resulting in small openings on the doors. Doors are also warped and do not self-close. 74SOLAS(20) II-2/4.2.1.

VESSEL RECEIVED HIGH TEMPERATURE ALARM ON THE FORWARD STERN TUBE BEARING WHILE ENROUTE TO PORT EVERLADES AND HAD TO REDUCE PROPULSION TO 1/2 POWER.

A fixed gas fire-extinguishing system complying with the provisions of the fire safety systems code. The means of control of any fixed gas fire extinguishing system shall be readily accessible and simple to operate. In CO2 room, control box for operation of release of CO2 gas to engine room brackets are broken from bulkhead and control box is suspended by ratchet strap secured to CO2 piping above. Additionally, arrangement of strap makes control box incapable of opening. SOLAS 74(14 CONS)II-2/10.4.1.1.1 FSS CODE CH 5.2.1.3.3
