The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulation to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. In order to receive a COI the vessel must conform to all conditions and regulatory requirements applicable to it. The vessel did not have a valid Vessel Security Plan, Merchant Mariner Credentials and TWIC’s, Shipboard Marine Pollution Emergency Plan, Safety management Certificate, Certificate of Financial Responsibility, Navigation Publications. Station Bill, Fire and Safety Plan, Ballast Water Management Plan, Waste Management Plan were not valid/missing. The vessel did not have a crew on board and could not conduct drills. As a result of an absent vessel security plan and crew, the vessel did not receive the ISSC. In order to conduct commercial operations under its issued COI, as a US flagged ship, the vessel will be required to show all documents are present/valid and a credentialed crew is available to conduct drills and demonstrate proficiency.

The shaft panel was open with disconnected wiring.

The safety management certificate intermediate verification was not conducted and expired on Feb 2016. Ensure verification is complete.

The aft stern light is inop on primary and secondary mode.

The radio certs were not endorsed. Ensure FCC licensed technician conducts tests and endorses the certificate.

Independent tanks are not authorized for cargo until MSC approval is received.
