Shutoff valves. Shutoff valves must be installed in the fuel supply lines, one as close to each tank as practicable, and one as close to each fuel pump as practicable. Shut off valve on aft deck for fuel tanks was wasted and may detach from bulkhead. Repair valve and valve housing. 46 CFR 56.50-75(b)(4)

Electrical installations on vessels must ensure the protection of the crew and vessel from electrical hazards. A-Frame lighting wiring had salt water entry into the plug on the main deck. Repair prior to using A-Frame Lighting. Vessel is restricted from conducting night time operations unless adequate lighting is provided to the aft deck. 46 CFR 111.01-1

Except as provided by § 132.100(b) of this subpart, each vessel must be equipped with one self-priming power-driven fire pump capable of delivering a single stream of water from the highest hydrant, through the hose and nozzle at a Pitot-tube pressure of at least 345 kPa. Vessels fire pump for fire monitor was leaking at the packing gland. 46 CFR 132.120

After loading but before departure, and at other times necessary to assure the safety of the vessel, the master shall verify that the vessel complies with requirements in its trim and stability book, stability letter, certificate of inspection and load line certificate, whichever apply, and then enter a statement of the verification in the logbook. Vsl missed their annual survey on the International Load line Certificate.

PAM's fwd ventilation does not close, must fix and prove proper operation with CGMI

Provide servicing report for additional immersion suits.(4)

Provide to CGMI an approved PAM and Electrical install plan.

Wire to temp MSD needs a better arraingement to prevent electrical haz and do not use zip ties.

Wires must terminate in an enclosure, port side engine room escape.

Light fixture must have cable gland for wires.

Provide one additional fire extiguisher for aft protable gen set.

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work except as modified by § 91.45–1. Vessel's fire control plan is not approved.

Electrical installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. Engine room distribution panel is missing blanks and/or circuit breakers in the panel, creating an electrical hazard.

Indication of the rudder angle must be provided both at the main steering station in the pilothouse and in the steering-gear compartment. The rudder-angle indicator must be independent of control systems for steering gear. Rudder angle indicator on the port rudder is not accurate. If your voyage time is less than 96 hours then you must submit an NOA before departure but at least 24 hours before arriving at the port or place of destination. Vessel was found arrived in Provport 08AUG2023 without submitting an ANOA. Vessel was contacted and ANOA was submitted after arrival. 33CFR160.212(a)

The ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. They shall also be permanently marked. Paint load line and ensure letters are permanently marked prior to crossing the boundary line.

Cylinders shall be so mounted as to be readily accessible. CO2 room was being used for storage and not accessible. Configure shelving in CO2 room to prevent equipment from impeding the access to the CO2 bottles and local controls.

Each lifebuoy must be capable of being rapidly cast loose. Relocate port and starboard lifebuoys on main deck from underneath the stairs so they are readily accessible.

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port. Clearly mark the port aft lifebuoy on the main deck.

Each vessel must carry lifebuoys approved under approval series 160.150. Replace retro on aft deck lifebuoys.

THIS SUBCHAPTER IS APPLICABLE TO ALL U.S. FLAG VESSELS INDICATED IN COLUMN 4 OF TABLE 2.01-7(a). VESSEL HAS BEEN OPERATING AS A LIGHTERING SUPPORT VESSEL, FENDERING FOR STS OPERATIONS. VESSEL IS CERTIFICATED UNDER 46 CFR SUBCHAPTER "L" AND THEREFORE CANNOT CONDUCT SUCH OPERATIONS WITHOUT BEING CERTIFICATED UNDER 46 CFR SUBCHAPTER "I". VESSEL IS GRANTED 90 DAYS TO OBTAIN CERTIFICATION UNDER 46 CFR SUBCHAPTER "I" TO SATISFACTION OF ATTENDING USCG MARINE INSPECTOR.

46CFR58.25-10 (b)The main steering gear and rudder stock must be- (1)Of adequate strength for and capable of steering the vessel at maximum ahead service speed, which must be demonstrated to the satisfaction of the cognizant Officer in Charge, Marine Inspection; (2)Capable of moving the rudder from 35 degrees on either side to 35 degrees on the other with the vessel at its deepest loadline draft and running at maximum ahead speed, and from 35 degrees on either side to 30 degrees on the other in not more than 28 seconds under the same conditions. While in transit offshore, steering gear/system became inoperable (lost rudder) and M/V JOSEPHINE K MILLER returned to the AET dock via assist T/V JUPITER. Must install new rudder and conduct sea trials with USCG attending MI prior to being placed back in service.

Vessel must have adequate emergency lighting fitted along line of escape to main deck. Both 6 person berthing spaces have no emergency lighting illuminating thr emergency hatch scuttles.
