Each OSV carrying NLSs in bulk must have a Cargo Record Book. Vessel was unable to locate an updated Cargo Record Book. Vessel has placed a Cargo Record Book into service.

Main inlet and outlet of of ventilation systems shall be capable of being closed from outside the space. Noted that the vent closures outside EDG space could not be completely closed and closure devices not aligned. 74SOLAS(14) II-2/ 5.2.1.1

An efficient bilge pumping system must be provided. Crew was unable to demonstrate bilge pumping arrangement. 74SOLAS(14) II-1/35

An alarm system shall be provided for any fault requiring attention. The oily waste tank high level is in alarm and the soundings reflect the tank is half capacity. 74SOLAS(14) II-1/51

The means to control any fixed gas extinguishing system shall have clear instructions relating to the operation of the system. The CO2 system for the paint locker instruction placard is ineligible and the activation cabinets for all CO2 systems have conflicting instructions for system activation. 74SOLAS(14) II-2/4.1 FSS Code Chapt 5/2.1.3.3

The rating or setting of an overload device for each circuit shall be indicated at the location of the device. Circuit directories do not match installed breakers. 74SOLAS(14) II-1/45.6.2

The general emergency alarm system required by III/6.4.2 must be audible throughout all the crew normal working spaces. The ship's general alarm is not audible in the engine room. (SOLAS III/6.4.3)

Means of escape from machinery spaces must be provided and maintained in a safe condition per 74 SOLAS (04) II-2/13.1. The emergency escape hatches from the port engine room, the starboard engine room, and the rudder room to the main deck could not be opened from inside the space. Additionally, the rudder room escape trunk was blocked by ship stores/parts. Provide and prove safe means of escape in accordance with 74 SOLAS (04) II-2/13.1.

Machinery space openings in position 1 or 2, and external openings in cargo ships leading to compartments assumed intact in the damage analysis are required to be watertight per 66/88 ICLL Annex I/17 and 74 SOLAS(04) II-1/25-10 respectively. Additionally, the ship shall be maintained in compliance with the requirements of its recognized classification society per 74 SOLAS (04) II-1/3-1. The closure device for the rudder room escape hatch was found seized in the open position. The port engine room escape hatch had a hole (approx. 5/8" x 1") in its coaming, and gaps were visible in the way of the closure gasket when secured and viewed from the inside. Make machinery space hatches to the weather deck operable and watertight.

Vessel crew was insufficiently knowledgeable of the vessel's specifics having been assigned only the night before. required Vessel to conduct and provide records of alll required drills to coast guard prior to departure from port, to be cleared within 5 days.
