Whenever there is a hazardous condition either onboard a vessel or caused by a cessel or its operation, the owner, agent, master, operator, or person in charge must immediately notify the nearest Coast Guard Sector office or group office... PSCO observed that the vessel failed to report missing starboard anchor on their notice of arrival. Vessel is required to submit an updated ENOA reflecting the lost anchor.
