-Ships of all types shall be subdivided into spaces by thermal and structural divisions having regard to the fire risk of spaces. MI found roll down waiter fire door in galley did not have current service report. 74 SOLAS (2004) II-2/2.1

To discharge ballast water into waters of the United States, the master, owner, operator, agent, or person in charge of a vessel subject to § 151.2025 of this subpart must either ensure that the ballast water meets the ballast water discharge standard as defined in § 151.2030(a), use an AMS as described in § 151.2025(a)(3) or ballast exclusively with water from a U.S. public water system, as described in § 151.2025(a)(2), according to the schedule in paragraph (b) of this section. Vessel was given a one time waiver to discharge ballast water at Chevron berth 3 Pascagoula, MS utilizing the IMO mode. Ensure all ballast water tanks are cleaned of all sediments that are prohibiting the proper functioning of the ballast water treatment system and provide proof of proper operation of the ballast water treatment system prior to ballasting operations, to the satisfaction of the vessels ACP RO (ABS).

Piping systems for ships must be designed, constructed, and inspected in accordance with ASME b31.1. Vessel had crack in the main hydraulic line for the cargo pumps. Complete repairs satisfactory to vessel's class society (ABS). Vessel is restricted from cargo operations that utilize the affected hydraulic system until a class report is provided to our office at [email withheld].

Lifesaving appliances and arrangements shall be approved by the Administration. Multiple PFDs were found in service that are approved for domestic routes, but not approved for international routes and service as required. 74 SOLAS (20) III/4.1

Structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these division shall be kept in good order so as to ensure their required performance if a fire occurs. Fire boundary penetrations in the deck and bulkheads of the navigation bridge electrical equipment room have mixed and disturbed fire stop material. All were found to have mismatched systems using a mix of concrete fire stop material and putty fire stop material in each penetration which is not an approved installation. 74 SOLAS (09) II-2/14.2.1.1.1

Each survival craft shall be stowed fully equipped as required. The inventory for both life boats had expired first aid kits and anti-seasickness medicine. 74 SOLAS (20) III/13.1.4 LSA IV/4.4.8

No person may operate or cause the operation of a vessel unless the vessel has the marine charts, publications, and equipment as required by 33 CFR parts 164.33 through 164.41. All of the vessel's nautical publications were out of date.

Each vessel must carry a Bridge-to-Bridge radio certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. The bridge-to-bridge certificate was endorsed with an incorrect call sign. 47 CFR 80.1005

The Oil Record Book Part 1 shall be completed on each occasion whenever bunkering of fuel or bulk lubricating oil takes place in the ship. Bunkering of fuel for the machinery operation was not recorded in the ORB Part 1 as required. MARPOL Annex I/17.2.5
