Instrumentation shall be fitted for continuously indicating and permanently recording, when inert gas is being supplied: 1. the pressure of the inert gas supply mains forward of the non-return devices required by paragraph 2.3.1.4.1; and 2. the oxygen content of the inert gas in the inert gas supply mains on the discharge side of the gas blowers. The inert gas generator (IGG) deck pressure interface displayed inconsistent readings between the local gauge and the control unit located in the cargo control room. It was noted that wiring to the pressure sensor (sending unit) had previously melted and was recently replaced. The crew repaired a small segment of wiring; However, the remaining length of wiring appears to be in questionable condition. Required Corrective Action: A root cause analysis shall be conducted to determine the cause of the original wiring damage and to ensure long-term reliability of this system for future cargo operations. Confirmation shall be provided from an authorized third-party technician or OEM technical support verifying that repairs to the pressure sensing unit and associated wiring are satisfactory and in full compliance with the manufacture's specifications. The vessel’s Safety Management System (SMS) shall document this occurrence as a reportable incident. Per 46 CFR 4.05-1(a)(4), any future incidents affecting the vessel’s fitness for service — particularly involving equipment vital to safe cargo operations — shall be immediately reported to the nearest USCG Officer in Charge, Marine Inspection (OCMI). SOLAS II-2/5.5.1.1 FSS Code Ch. 15.2.4.2.1
