PSCO observed inoperable IGG system in accordance with the requirements of the Fire Safety Systems Code. Upon expansion, PSCO identified that vessel discharged cargo on 28MAY22 without utilization of IG. Vessel is restricted from cargo operations requiring IG.

Each self propelled ship must have a shower and eyewash fountain that operates at all times. PSCO discovered multiple leaks on fresh water supply line which prevented the operation of shower and eyewash fountain. Some of the flanges were loose, other areas of pipes had wastage.

Each cargo ship shall have one or more totally enclosed lifeboats complying with the requirements of section 4.6 of the code. Every totally enclosed lifeboat shall be provided with a rigid watertight enclosure which completely encloses the lifeboat including hatches which can be closed to make the lifeboat watertight. Port side lifeboat access hatch is not watertight. Additionally, all three hatches on both lifeboats were replaced since last survey and have not been observed by class.

Drills as far as practicable shall be conducted as if they were an actual emergency. The required equipment, specifically the lifeline and flashlight were not brought to the fire muster station. Additionally, the crew did not bring enough length of fire house, leaving the fire team approximately 20 feet from the affected space. The fire fighter was not able to properly don the SCBA and mask, allowing air to escape and draining an entire SCBA bottle before they even reached the fire. Fire fighters did not properly access the affected space. One fire fighter took off his glove and put it on the door to the space.
