The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. A cracked weld was found on the PORT generator exhaust piping.

Piping used in a vital system must: be composed of ferrous materials except when nonferrous metallic piping materials are permitted by 182.730. The following def's were noted. Replace Port MDE raw water pipe due to numerous leaks Replace raw water pipe leading to the port main exhaust.

For the purpose of flexibility only, flexible hose installed in lengths of not more than 30 inches and subject to pressures of not more than 5 psig, may meet the following requirements: USCG type A1, A2, B1, or B2 flexible hoses may be accepted. The port fuel return line is not serviceable. Replace with J-1942 hose of any length or USCG type A1 hose of not greater than 30" if the system does not exceed 5 psig.

A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. The saltwater cooling pipe for the starboard jets passes through the aft engine room watertight bulkhead and is leaking at the bulkhead.

Piping, including valves, pipe fittings, and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 150 F, must be suitably insulated where necessary to prevent injuries. The starboard generator jacket water piping leading to the expansion tank was not covered to prevent injury.

Each vessel that operates on high seas must have EPIRB installed to automatically float free and activate. Vessel must register new EPIRB and proper installation w/ hydrostatic release.

Exhaust systems must ensure minimum risk of injury to personnel. Protection must be provided. Ensure protection on #1 and #3 MDE exhausts. Re secure bracket on #3 MDE exhaust.

Each ring buoy must be marked with capital letters. Re stencil life buoy lettering for vessel name and hailing port.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space...and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Vessel crew discovered a 1" x 1" puncture in the starboard side shell below the waterline in way of the generator room. Vessel is to transit directly to Diamond Services Shipyard in Amelia, LA to effect permanent repairs. Upon arrival, vessel is to contact MSU Morgan City Command Duty Officer at [phone withheld] and shall coordinate oversight of repairs prior to commencing any repair work.

SSG caught fire in rotor/stator, probable cause accumulation of residue/particulate/dust caused overheating and ignited

MOB Drill was poorly conduct. Deficient in communication, individual role, deployment of ladder at rescue zone, and urgency as if actual emergency

Hull/Exhaust interface has cracked weld seam on port side.
