For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Vessel's fixed fire CO2 system for engine room is overdue for servicing. Servicing was due in January 2026. Vessel operator was able to provide attending MI a risk mitigation plan for an engine room fire. Vessel is allowed to operate for 02 scheduled trips on 10MAY2026, in accordance with provided mitigation plan. Vessel is required to get fixed fire CO2 system serviced by end of 11MAY2026. Vessel will be restricted from carrying passengers after 11MAY2026, if fixed fire CO2 system is not serviced. 46 CFR 176.810(b)(2)

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. #1 A/C pump junction box was improperly mounted with zip-ties. 46 CFR 183.220(a)

"All griddles, broilers, and deep fat fryers must be fitted with a grease extraction hood..." MI observed deep fat fryer in galley space with no grease extraction hood or fixed fire extinguishing. Remove deep fat fryer. 46 CFR 181.405(b)

"Electrical equipment exposed to corrosive environments must be of suitable construction and corrosion-resistant." Starboard aft battery bank was observed with corroded terminal. Replace battery. 46 CFR 183.210(c)

"All cable and wire must: be protected from the weather." Bilge pump wiring in lazarette was observed without junction box. Add junction box. 46 CFR 183.340(b)(3)

"Electrical equipment on a vessel must be installed and maintained to: minimized accidental personnel contact with energized parts." MI observed dead end wiring in lazarette in the vicinity of hydraulic pump. Fix dead end wire. 46 CFR 183.200(c)

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Attending inspector found a photocopy of a COD with "VOID" watermark on it which indicates a copy. Outfit vessel with original COD. 46 CFR 67.313

Strong and effective rudder stops must be provided to prevent jamming and damage to rudder... Attending Marine Inspector identified cracked rudder stop on the starboard rudder. outfit vessel with fixed rudder stop. 46 cfr 182.610

Boundaries that separate machinery spaces from accommodation spaces, service spaces, and control spaces must be lined with noncombustible panels or insulation. Attending MI identified missing insulation over starboard engine. Outfit space with missing insulation. 46 cfr 177.410

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable. Attending MI identified missing clamp on starboard wet exhaust. Outfit exhaust with double clamp. 46 cfr 182.410

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Identified space heaters and dehumidifiers in engine room. Remove from vessel. 46 cfr 176.830

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Attending inspector found leaks on the hydraulic fittings for the steering system. Outfit vessel with means to address/fix hydraulic leak. 46 cfr 182.100

A hatch exposed to the weather must be watertight. Identified starboard steering gear hatch gasket to be ineffective at keeping water at bay. 46 cfr 179.360

A vessel with a power driven main steering gear must be provided with the following: a manual means to center and steady the rudders in an emergency. Attending inspector identified no means to center or steady the rudder. Provide proposal to Coast Guard for means of rudder centering. 46cfr182.610

Each vessel subject to 46 CFR subchapter T must be marked with the vessel name on some clearly visible part of the port bow, starboard bow and stern. Markings must be over 4 inches in height. Vessel still had markings for previous name. Vessel movement is prohibited prior to application of correct vessel name markings except for a single transit without passengers to the drydock facility where lettering will be applied. Vessel Traffic Service must be notified at [phone withheld] prior to the vessel getting underway for this transit. 46 CFR 185.602

Complete but simple instructions for operation are required to be posted near operating equipment for fixed CO2 systems. The time delay unit on the vessel's CO2 system did not have a label specifying the 30 second time delay. 46 CFR 185.612 (disregard cite below)

A hatch cover must be attached to the hatch frame or coaming by hinges, captive chains , or other devices of substantial strength to prevent its loss. The hatch to the steering gear space did not have a means of retention. 46 CFR 179.350

Floating waterlights must be designed to meet the requirements of UL 1196. The battery in the main deck aft life ring waterlight had a battery that did not appear to be designed for the waterlight. Replace with manufacturer recommended battery type. 46 CFR 180.71

Each ring life buoy must be clearly marked with the vessel's name and type I retro-reflective material. The port bow ring life buoy was still marked with the vessel's previous name and applied retro-reflective material was worn. 46 CFR 185.604

Vessels over 65 feet must have at least one fire axe located adjacent to the operating station. The axe on the vessel was still labeled with the previous name.

This vessel has not conducted a DD/ISE inspection to maintain a valid COI. COI has been revoked. The vessel shall be made available for a Coast Guard inspection immediately prior to commercial operation.

Certificate of Documentation is expired. Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under §67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. Provide proof of valid Certificate of Documentation.

Pre-employment testing requirements. Vessel is currently not operating and therefore has no crew. (a) No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Prior to operating vessel with passengers on board, crewmembers must provide proof of a valid, negative pre-employment drug test and be enrolled in random drug testing.

Vessel was granted a Letter of Deviation from the AIS requirement outlined 33 CFR 164.46 for 5 years beginning on 10DEC15. LOD expired on 10DEC20, but vessel has not operated since Feb 2020. Prior to resuming operations, owner/operator must install an AIS or request a new Letter of Deviation.

The small pressure vessel in void 3 is under 5 cubic feet & is not stamped with ASME “U” or “UM” symbol. All unfired pressure vessels must be installed to the satisfaction of the cognizant [name withheld] vessels must meet applicable requirements of Subchapter F of 46 CFR 54.01-5(c)(4) - Classes II & III pressure vessels that have a net internal volume of less than 0.14 cubic meters (5 cubic feet) and do not contain hazardous materials must be stamped with either the ASME “U” or “UM” symbol. Replace pressure vessel with pressure vessel that complies with ASME standard & stamped accordingly.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in a space with a through-hull fitting below the deepest load waterline, such as a lazarette. The bilge alarm for this space is in continuous alarm therefore compartment experienced a true high water level, master would not know. Repair high level bilge alarm to operate properly.

Each floating waterlight installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the waterlight to be quickly disconnected from the ring life buoy. The lanyard was severely weathered and frayed. Line was replaced prior to end of inspection.

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. The bilge suction in the lazarette was missing the strainer. Vessel is currently not operating this season. Replace missing strainer with one of appropriate size prior to operating vessel.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel, and at least once every three months as to the duties that the crewmember is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard by 46 CFR 185.510. Vessel is not operating this season. Perform required training and drills prior to carrying passengers.

The fixed fire extinguisher system 30 second time delay actuated in 47 seconds. Replace time delay bottle to the satisfaction of the attending USCG Marine Inspector.

Expired first aid items.

Three orange smoke flares were expired.

Automatic heat detectors in engine room did not properly activate CO2 release mechanism.

Aft generator raw water pump was leaking at shaft seal.
