THE DEFICIENCIES ABOVE PROVIDE OBJECTIVE EVIDENCE OF A SERIOUS FAILURE OF THE IMPLEMENTATION OF THE ISM CODE. THE EXTERNAL AUDIT REPORT SATISFACTORY TO THE COTP MUST BE SUBMITED PRIOR TO CLEARANCE OF THE DEFICIENCY.

The system shall also be provided with arrangements to ensure that any discharge of oily mixtures is automatically stopped with the oil content of the effluent exceeds 15 ppm.
