Portside liferaft was tied to the ship directly and would be dragged under by the sinking ship. starboard liferaft painter was not attached to anything nor will inflate as intended. 74 SOLAS (14)III/13.4.2 & LSA CODE (2017)4.1.6

0725- FIXED FIRE EXTINGUISHING INSTALLATION- Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed the seawater isolation valve for the fixed foam system in the closed posistion. Fixed fire supression system could not be engaged from a remote location as designed. In addition, PSCO observed the fixed water based extinguishing system was on manual mode. Vessel was approved for periodically unattended machinery space. 74 Solas (2004) II-2/14.2.1.2

Hydrostatic release unit for both liferafts were not marked with the expiration date. Crew had no correspondence linking the HRU with expiry dates. HRU was not fitted with a weak link.
