The alarms required in paragraphs 2.2.4.5.1.1, 2.2.4.5.1.3, and 2.2.4.5.1.5 shall be fitted in the machinery and cargo control room, where provided, but in each case in such a postion that they are immediately recieved by responsible members of the crew. PSCO observed absence of audible and visual alarms for the inert gas panel located in the CCR. 74 SOLAS (2004) II-2/4.5.5.1.1 FSS 2.2.4.5.2

Detailed instruction manuals shall be provided on board, covering the operations, safety and maintenence requirements and occupational health hazards relevant to the inert gas system and its operation to cargo tank system. The manuals shall include guidance on procedures to be followed in the event of fault or failure of the inert gas system. PSCO observed vessels IGG operation manual indicates the main control panel is located in the engine control room, however, the main control panel is located near the IGG near the upper platform of the engine room. Additionally, PSCO observed modifications to the systems. The CCR panel now contains a 3 pen digital recorder where the IGG operations manual mentions a 2 pen recorder. 74 SOLAS (2004) II-2/4.5.5.1.1 FSS 2.2.5

Electrical equipment, cables, and wiring shall not be observed in the hazardous areas, unless it conforms with the standards not inferior to these acceptable to the Organization. PSCO observed a fault on the 5P isolation barrier for the Kongsberg tank radar system. The crew is unable to identify the cause of the fault. 74 SOLAS (2020) VII/10.1 IBC 10.1.4

Ship must have a high level alarm that gives an audible and visual alarm before the tank reaches 97%. The visual alarm from the deck is not operational. Prior to loading cargo, the alarm must be operational in the United States. 46CFR153.409

2470-Notification-While ships are operating within an ECA, the sulfur content used onboard shall not exeed 1%. The PSCO discovered the vessel has been operating within the North American ECA from 06/02/2014 to 06/04/2014 utilizing fuel oil that does not comply with MARPOL Annex VI ECA sulfur limits.-MARPOL/VI.14.4

0611-Lifeboat Inventory-All life-saving appliances shall be in working order and ready for immediate use. Nine of the vessel's 34 food rations were not air-tight. Expiration of all food rations is June 2014. Vessel has ordered food rations for this port. Provide proof of receipt no later than 06/06/2014.-SOLAS(09 ConEd) III/20.2

1840-Instrumentation-Tankers shall be equipped with at least one portable instrument for measuring flammable vapor concentrations. Suitable means shall be provided for the calibration of such instruments. Vessel does not have means to calibrate portable gas detection meters. Vessel has ordered gas for calibration for this port. SOLAS (09ConEd) II-2/4.5.7.1

Make repairs to the oily water separator oil content meter. Meter does not report as required by MEPC (107). Meter does not read the number '0' during the clean water test. Until corrected, oil residues to be retained on board for discharge ashore. Deficiency is to be rectified prior to next US port after sailing foreign to the satisfaction of Class.
